摘要
陈建明教授认为,不寐病位在心、肝,七情内伤、劳神过度是其主要病因,临证时常灵活运用费伯雄的《医醇賸义》所载方药,多从心肝辨证论治。郁火扰心证以清火解郁、养血宁心为治疗大法,方选解郁合欢汤;心火亢盛证以清心泻火、养阴安神为治疗大法,选用加味泻心汤加减;心气不足证以调补营卫、安养心神为治疗大法,方选宅中汤加减;心肝血虚证以养血安神、疏肝解郁为治疗大法,方选甲乙归藏汤加减。陈教授强调,在辨证用药时应心肝同治,顺应脏腑气机功能,同时注重调护,注意劳逸结合,自我调整情绪,避免熬夜、饮用浓茶、酒和咖啡等,以达到更好的效果。
Professor Chen Jian-ming believes that insomnia is located in the heart and liver.Internal injury of seven emotions and overwork are the main causes of insomnia.In clinical practice,we often flexibly use the formulas and drugs contained in Fei Bo-xiong′s The Refined in Medicine Remembered,mostly from the syndrome differentiation and treatment of heart and liver.For the syn-drome of stagnant fire disturbing the heart,clearing fire and relieving depression,nourishing blood and calming the heart are the main treatment methods,so Depression-Removing Flos Albiziae Decoction can be used.For syndrome of hyperactivity of heart fire,clearing the heart and purging fire,nourishing yin and calming the nerves are the main treatment methods,so.Modified Supplemen-ted Heart-Draining Decoction can be used.For syndrome of heart qi deficiency,regulating and supplementing nutrient and defen-sive qi,calming the mind are the main treatment methods,so Modified Zhaizhong Decoction(宅中汤加减,MZZD)can be used.For syndrome of heart-liver blood deficiency,nourishing blood and calming the nerves,soothing the liver and relieving depression are the main treatment methods,so Modified A and B Essence-Restoring Decoction can be used.Professor Chen stresses that when using drugs based on syndrome differentiation,it is necessary to treat both the heart and the liver,comply with the functions of vis-cera and qi movement,pay attention to nursing,pay the combination of work and rest,self-adjusting emotions,and avoid staying up late,drinking strong tea,wine and coffee,so as to achieve better efficacies.
