摘要
目的:探讨128层动态容积CT全瘤灌注成像在肺内孤立性肿瘤患者良恶性鉴别诊断的价值。方法:选取2020年7月—2022年1月就诊于南充市高坪区人民医院的100例肺内孤立性肿瘤患者,采取128层容积C T单入口、双入口联合模式全瘤灌注参数检查,金标准为病理诊断,分析良恶性鉴别准确度。结果:经检测,100例患者中金标准检查结果为40例为肺内孤立性癌(恶性),剩余60例为肺内孤立性肿瘤(良性)。良恶性患者PI、AF、S-AF、E.BV具有显著差异(P<0.05);相比单纯形态学诊断以及单一灌注参数诊断,多参数联合检查并结合形态学诊断阳性诊断值高达97.50%,阴性诊断值也达到95.00%,诊断效能较为显著。结论:对肺内孤立性肿瘤患者进行良恶性鉴别时,实施128层容积CT单入口、双入口联合模式全瘤灌注成像检查准确度较高,临床医师能够对患者肿瘤及其周边情况有更为全面的了解,临床价值显著。
Objective To evaluate the value of 128-slice dynamic volumetric CT perfusion imaging in differential diagnosis of benign and malignant solitary lung tumors.Methods From July 2020 to January 2022,100 patients with isolated pulmonary tumors admitted to The People's Hospital of Gaoming District,Nanchong City were selected,and the whole tumor perfusion parameters were examined by 128-slice CT combined with single inlet and double inlet modes.The gold standard was pathological diagnosis,and the accuracy of differentiating benign and malignant tumors was analyzed.Results Among the 100 patients,40 were isolated lung cancer(malignant)and the remaining 60 were isolated lung tumor(benign).There were significant differences in PI,AF,S-AF and E.BV between benign and malignant patients(P<0.05);Compared with the simple morphological diagnosis and the single perfusion parameter diagnosis,the multi-parameter combined examination combined with the morphological diagnosis had a positive diagnostic value of 97.50%and a negative diagnostic value of 95.00%,indicating a significant diagnostic efficiency.Conclusion When differentiating benign and malignant patients with isolated lung tumors,the 128-slice volume CT single inlet and double inlet combined mode of whole tumor perfusion imaging has high accuracy,and clinicians can have a more comprehensive understanding of the patient's tumor and its surrounding conditions,with significant clinical value.
