摘要
目的评价加速康复理念下老年股骨粗隆间骨折患者围术期规律性应用氟比洛芬酯临床疗效。方法回顾性分析60例股骨粗隆间骨折手术患者的临床资料,根据是否规律性应用氟比洛芬酯分为观察组和对照组,每组30例,在加速康复外科理念进行术后教育及护理的基础上观察组术后规律使用氟比洛芬酯,对照组给予地佐辛镇痛,分析两组术后VAS评分,镇静评分(RSS评分法),睡眠质量评分(PSQI评分法),术后首次下床时间、平均住院日、平均总住院日及术后药物不良反应发生率。结果观察组术后48 h内VAS≥4分时疼痛的发生率、首次下床时间(2.37±0.56 d)、平均住院日(5.50±1.17 d)、总平均住院日(7.67±0.96 d)、不良反应发生率(6.06%)均低于对照组(P<0.05),术后48 h内PSQI评分要低于对照组,而术后24 h内RSS评分要高于对照组。结论在ERAS理念下老年股骨粗隆间骨折患者,术后规律性应用氟比洛芬酯组临床疗效好,药物的不良反应低。
OBJECTIVE To evaluate the clinical efficacy of regular perioperative flurbiprofenaxetil injection for elderly patients with intertrochanteric fracture under the concept of enhanced recovery after surgery(ERAS).METHODS Clinical data of 60 patients with intertrochanteric fracture of femur undergoing surgery were retrospectively analyzed.Patients were divided into observation group and control group according to regularity of flurbiprofen application.There were 30 cases in each group.The observation group was treated with flurbiprofenaxetil injection regularly after surgery,while the control group was treated with desoxocin.According to the concept of enhanced recovery after surgery,postoperative education and nursing were provided to both groups.The postoperative VAS score,sedation score(RSS score)and sleep quality score(PSQI)of the two groups were observed,and the first leaving bed time,the average postoperative hospital stay,the average total postoperative hospital stay and the incidence of postoperative adverse reactions of the two groups were compared.RESULTS The incidence of pain,the first leaving bed time(2.37±0.56 d),average hospital stay(5.50±1.17 d),total average hospital stay(7.67±0.96 d)and the incidence of adverse reactions(6.06%)in the observation group were lower than those in the control group(P<0.05).The PSQI score was lower in 48 hours after operation,but the RSS score was higher in 24 hours after operation.CONCLUSION Under ERAS era′s concept,the group of elderly patients with intertrochanteric fractures who were routinely treated with flurbiprofen axetil had good clinical efficacy and low adverse drug reactions.
