摘要
目的探讨加味涤痰汤介入湿性年龄相关性黄斑变性(AMD)临床治疗疗效及对视网膜后极部CNV面积、黄斑中心凹视网膜厚度(CRT)、血清VEGF水平的影响。方法选取2018年1月至2021年6月我院收治的70例(72眼)湿性AMD患者,按照随机数字表法分为对照组和观察组各35例(36眼)。两组患者均使用康柏西普玻璃体腔注射及视网膜激光光凝术常规治疗,观察组在原治疗基础上使用加味涤痰汤。对比两组患者治疗效果,CNV、CRT、VEGF变化及并发症发生率。结果治疗后,观察组的总有效率为92.72%,高于对照组的77.78%(P<0.05)。治疗1个月后,两组CRT、VEGF水平比较,无显著性差异(P>0.05);治疗2、3个月后,观察组CRT、VEGF水平低于对照组(P<0.05)。治疗1个月后,两组CNV比较,无显著性差异(P>0.05),治疗2、3个月后,观察组CNV小于对照组(P<0.05)。两组并发症发生率比较,无显著性差异(P>0.05)。结论加味涤痰汤介入治疗湿性AMD可降低患者血清VEGF水平,改善视网膜后极部CNV面积和CRT厚度,临床疗效更好,且并发症发生率低。
Objective To investigate the curative effect of Flavored Ditan Decoction in the clinical treatment of wet age-related macular degeneration(AMD)and itsinfluences onthe CNV area of the posterior pole of the retina,the central foveal retinal thickness(CRT),and serum VEGF level.Methods We select 70 patients(72 eyes)with wet AMD admitted to our hospital from January 2018 to June 2021 as research objects,and divide them into control group and observation group randomly,with 35 cases(36 eyes)in each group.Both groups were treated with conventional treatment of conbercept intravitreal injection and retinal laser photocoagulation,and the observation group were treated withFavored DitanDecotion on the basis of original treatment.The therapeutic effect,changes of CNV,CRT,VEGF and incidence of complications were compared between the two groups.Results After treatment,the total effectiveness rate of the observation group was 92.72%higher than that of the control group(P<0.05).After one month of treatment,the difference in CRT and VEGF levels between the two groups were not statistically significant(P>0.05),and after 2 or 3 months of treatment,CRT and VEGF levels in the observation group weresignificantly lower than those in the control group(P<0.05).After one month of treatment,the difference of CNV between the two groups was not statistically significant(P>0.05),and after 2 or 3 months of treatment,CNV level in the observation group was significantly lower than that in the control group(P<0.05).There was no significant difference in complication rates between the two groups(P>0.05).Conclusion The treatment of wet AMD with flavored DitanDecoction can reduce the serum VEGF level of patients,improve the CNV area and CRT thickness of the posterior pole of retina.The treatment achieves good clinical efficacy,and the complication rate is low.
