输尿管内支架管置入术和肾穿刺造瘘术处理结石并感染的对比 被引量:1

摘要 目的 探究输尿管内支架管置入术和肾穿刺造瘘术处理结石并感染的对比。方法 选取2019年1月-2020年6月于连州市人民医院行手术治疗的输尿管结石梗阻性尿脓毒症患者80例,据不同手术方案分为对照组与研究组,每组各40例。对照组行肾穿刺造瘘术,研究组行输尿管支架管置入术,观察分析两组感染指标血白细胞计数(WBC)、中性粒细胞比例(NEUR)、C反应蛋白(CRP)、降钙素原(PCT)水平、引流成功率及并发症情况。结果 两组感染指标WBC、NEUR、CRP、PCT水平术前组间比较,差异无统计学意义(P>0.05);术后研究两组术后感染指标WBC、NEUR、CRP、PCT水平分别术前组内比较均降低,差异有统计学意义(P<0.05)。研究组引流成功率高于对照组(P<0.05);研究组并发症发生率低于对照组(P<0.05)。结论 经分析输尿管镜输尿管支架管置入术较肾穿刺造瘘术的引流成功率高,且利于抽取肾积水,降低降钙素原水平,减轻脓毒症,避免病情进一步恶化,且安全性高,值得推广。 Objective To explore comparison between ureteral stent implantation and nephrostomy in treatment of stones and infection.Methods The paper chose 80 patients with ureteral stone obstructive urosepsis with surgery in our hospital from January 2019 to June 2020,and divided them into two groups according to different surgical schemes,control group and study group,with 40 cases in each group.Control group was treated with nephrostomy and study group with ureteral stent implantation.Infection indexes between two groups were observed and analyzed:blood leukocyte count (WBC),neutrophil ratio (neur),C-reactive protein (CRP),procalcitonin (PCT),drainage success rate and complications.Results Postoperative levels of WBC,neur,CRP and PCT in study group;success rate of drainage in study group was 100.00%,higher than 85.00% in control group (P<0.05);incidence of complications in study group was 2.50%,lower than 12.50% in control group (P<0.05).Conclusion After analysis,ureteroscopic ureteral stent implantation has higher drainage success rate than nephrostomy,can extract hydronephrosis,reduce procalcitonin level and sepsis,avoid further deterioration of the disease with high safety,which is worthy of promotion.
