摘要
目的神经元特异性烯醇化酶(NSE)、叶酸(FA)、胰岛素(INS)、甲状旁腺素(PTH)及促肾上腺皮质素(ACTH)的检测经常受到溶血的干扰,给临床检验带来较大困惑。现就标本溶血对这5项指标的检测干扰展开定量研究,并制备溶血标本比色卡,为临床标本接收与结果判读提供简便易行的参考依据。方法利用超声裂解红细胞,制备不同溶血程度(0%、0.1%、0.3%、1%、3%、10%)的血清样本,进行化学发光免疫法检测NSE、FA、INS、PTH、ACTH水平,并对结果进行相关性分析。再利用溶血标本肉眼颜色外观及各溶血程度下的干扰值制备溶血图表。结果随溶血程度增加,NSE与FA的检测值依次升高,而INS、PTH与ACTH的检测值则依次降低。统计学分析均显示非常显著的干扰。NSE与FA的结果绝对增加量与INS、PTH与ACTH的结果抑制程度,分别与溶血程度呈高度线性关系,并因此推算出结果校正公式。结论血清溶血标本参考比色图可为临床检测工作中NSE、FA、INS、PTH及ACTH的标本接收及结果判读提供简便直观的参考依据。类似溶血干扰研究与图表制备可用于其他受溶血影响的临床指标检测中。
Objective Blood hemolysis often interferes with clinical tests,which imposes a great challenge on clinical staffs in hemolytic sample acceptance and result interpretation.In the current study,we aimed to quantitatively analyze the hemolysis interference on five commonly affected tests,including neuron specific enolase(NSE),folic acid(FA),insulin(INS),parathyroid hormone(PTH)and adrenocorticotropic hormone(ACTH);and also prepare a simple reference chart for the frontline work of clinical laboratories.Methods Endogenous red blood cells were sonicated to prepare hemolyzed serum samples at different degrees(0%,0.1%,0.3%,1%,3%,10%).Chemiluminescence immunoassays of NSE,FA,INS,PTH and ACTH were performed.Correlations of the results with hemolysis were then analyzed.The apparent color and quantitative results of hemolyzed serum samples were summarized based on images and with a table as a reference chart for convenient lookup.Results Hemolysis showed an additive effect on NSE and FA tests,while exerted inhibitory effect on INS,PTH and ACTH testing,all in hemolysis degree-dependent manner with highly linear relationships and significant differences.The reference chart demonstrated visible colorimetric scales for the estimation of hemolysis degrees and provided simple derivation of actual analyte levels.Conclusion The reference chart provides a simple criterion for hemolysis sample acceptance and result interpretation.Similar approaches can be developed with other hemolysis-interfered clinical tests.
