摘要
目的探究老年肺结核患者抗结核治疗前后血清及痰液miR-144的变化情况。方法选取2017年5月~2019年4月收治的90例老年肺结核患者作为试验组,包括30例痰菌阳性伴空洞患者(A组)、30例痰菌阴性伴空洞患者(B组)及30例单纯痰菌阴性患者(C组),予以抗结核治疗,再选择同期入院的30例上呼吸道感染患者为对照组,对比治疗前后血清及痰液miR-144的表达情况。结果治疗前,试验组血清miR-144表达及痰液miR-144表达均高于对照组,差异有统计学意义(P<0.05);抗结核治疗后,试验组血清miR-144表达及痰液miR-144表达均低于治疗前,且高于对照组,差异均有统计学意义(P<0.05)。治疗前,A、B、C三组血清及痰液miR-144表达均高于对照组(P<0.05),A组血清及痰液miR-144表达高于B、C组(P<0.05),B组高于C组(P<0.05)。治疗后A、B、C三组中病情好转患者的血清及痰液miR-144表达均低于治疗前(P<0.05)。结论肺结核患者血清及痰液miR-144呈高表达,伴空洞患者血清及痰液miR-144表达最高,抗结核治疗后病情好转患者血清及痰液miR-144明显降低,血清及痰液miR-144表达可视作肺结核抗结核治疗后评价病情转归水平的重要标志物。
Objective To explore the levels of miR-144 in serum and sputum of older patients with pulmonary tuberculosis before and after anti-tuberculosis treatment.Methods Ninety older patients with pulmonary tuberculosis from May 2017 to April 2019 were selected as the experimental group,including 30 sputum positive cases with cavity(group A),30 sputum negative cases with cavity(group B)and 30 simple sputum negative cases(group C).They received anti-tuberculosis treatment.Thirty patients with upper respiratory tract infection admitted in the same period were selected as the control group.The expression of miR-144 in serum and sputum before and after treatment were compared.Results Before treatment,serum and sputum miR-144 levels in the experimental group were significantly higher than those in the control group(P<0.05).After anti-tuberculosis treatment,serum and sputum miR-144 levels in the experimental group were lower than those before treatment and higher than those in the control group(P<0.05).Before treatment,serum and sputum miR-144 levels in group A,B and C were higher than those in the control group(P<0.05).Serum and sputum miR-144 levels in group A were higher than those in group B and C(P<0.05),and were higher in group B than those in group C(P<0.05).Serum and sputum miR-144 levels in patients with improved condition in group A,B and C were lower than those before treatment(P<0.05).Conclusion The levels of miR-144 in serum and sputum of patients with pulmonary tuberculosis are high,and are highest in patients with cavity.After anti-tuberculosis treatment,the levels of miR-144 in serum and sputum of patients with improved condition are significantly reduced.Serum and sputum miR-144 levels can be regarded as an important marker to evaluate the prognosis of pulmonary tuberculosis after anti-tuberculosis treatment.
