摘要
目的分析老年慢性阻塞性肺疾病急性加重期合并呼吸衰竭患者开展呼吸训练联合无创正压通气治疗对肺通气状态的影响。方法选择无锡市第五人民医院在2019年至2021年期间收治的老年慢性阻塞性肺疾病急性加重期合并呼吸衰竭患者105例为研究对象,随机单盲法分组,对照组52例实施无创正压通气治疗,观察组53例联合呼吸训练,比较两组患者治疗效果、各项血气指标、体温恢复正常时间、憋喘消失时间、肺部啰音消失时间、住院时间。结果观察组患者的治疗有效率较对照组高,动脉血二氧化碳分压、呼吸困难指数较对照组低,动脉血氧饱和度、动脉血氧分压较对照组高,体温恢复正常时间、憋喘消失时间、肺部啰音消失时间、住院时间较对照组短,差异有统计学意义(均P<0.05)。结论呼吸训练联合无创正压通气治疗能显著提升慢性阻塞性肺疾病急性加重期合并呼吸衰竭患者的疗效,改善患者的肺功能和血气相关指标,缩短病程时间。
Objective To analyze the effect of respiratory training combined with non-invasive positive pressure ventilation on pulmonary ventilation status in elderly patients with acute exacerbation of COPD complicated with respiratory failure.Methods 105 senile COPD patients with acute aggravating period in the Fifth People's Hospital of Wuxi during the year of 2019 to 2021 were selected,they were randomly and blindly divided into control group(n=52)and observation group(n=53),the control group received noninvasive positive pressure ventilation therapy,while the observation group accepted joint breathing exercises,the treatment effect,blood gas related index,time for body temperature to return to normal,the effect of asthma disappearance time,lung rales disappearance time and hospitalized time were compared between the two groups.Results The treatment efficiency were higher in the observation group than in the control group,the arterial blood CO_(2) partial pressure,dyspnea index were lower in the observation group than in the control group,the SPO_(2) and arterial blood oxygen partial pressure were higher in the observation group than the control group,time for body temperature to return to normal,breath disappear time,lung rale disappear time,hospitalized time were shorter(P<0.05).Conclusions Respiratory training combined with non-invasive positive pressure ventilation has a strong improvement effect on ventilation in elderly patients with acute exacerbation of COPD complicated with respiratory failure,which can improve lung function,significantly improve blood gas related index and shorten the duration of disease,which is worthy of application.
