摘要
目的:探讨关节镜下钢板内固定术联合植骨融合对胫骨平台粉碎性骨折患者膝关节功能的影响。方法:选取2019年5月-2021年5月张掖市第二人民医院接收的78例胫骨平台粉碎性骨折患者,依据不同术式分为对照组和观察组,各39例。对照组予以关节镜下钢板内固定术治疗,观察组予以关节镜下钢板内固定术与植骨融合联合方案治疗。对比两组的骨折复位情况、术后疼痛与恢复时间、不良事件发生情况及膝关节功能。结果:观察组的关节面塌陷、平台增宽、成角畸形评分均比对照组高(P<0.05);观察组术后7 d、3个月的视觉模拟评分法(VAS)评分均比对照组低,骨折愈合时间、完全负重时间均比对照组短(P<0.05);观察组的不良事件发生率为5.13%,比对照组的20.51%低(P<0.05);观察组术后6个月的美国特种外科医院膝关节评分(HHS)均比对照组高(P<0.05)。结论:关节镜下钢板内固定术与植骨融合联合方案用于胫骨平台粉碎性骨折患者,可提升复位、术后恢复效果,并降低不良事件发生率,有效改善膝关节功能。
Objective:To investigate the effect of arthroscopic plate internal fixation combined with bone graft fusion on knee function in patients with tibial plateau comminuted fractures.Method:A total of 78 patients with tibial plateau comminuted fractures were accepted by Zhangye Second People’s Hospital of from May 2019 to May 2021 were selected,according to different operation methods,they were divided into control group and observation group,39 cases in each group.The control group was treated with arthroscopic plate internal fixation,the observation group was treated with arthroscopic plate internal fixation combined with bone graft fusion.The fracture reduction,postoperative pain and recovery time,occurrence of adverse events and knee joint function were compared between the two groups.Result:The scores of articular surface collapse,platform widening,and angular deformity were in the observation group higher than those of the control group(P<0.05).7 d and 3 months after surgery,the visual analogue scale(VAS)scores of the observation group were lower than those of the control group,fracture healing time and full weight-bearing time were shorter than those of the control group(P<0.05).The incidence of adverse events was 5.13%in the observation group,which was lower than 20.51%in the control group(P<0.05).6 months after the surgery,the Hosptial for Special Surgery knee score(HHS)of the observation group were higher than those of the control group(P<0.05).Conclusion:The arthroscopic plate internal fixation combined with bone graft fusion for the treatment of patients with tibial plateau comminuted fractures can improve the reduction and postoperative recovery effect,reduce the incidence of adverse events,as well as effectively improve the knee joint function.
