The application of MALDI-TOF MS in rapid detection of carbapenem-resistant Klebsiella pneumonia and its homology analysis

摘要 目的利用基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)筛选碳青霉烯类耐药肺炎克雷伯菌(CRKp)特异性峰,并进行同源性分析。方法收集30株CRKp临床分离株,采用法国梅里埃VITEK 2 Compact分析仪进行细菌鉴定和药敏试验、利用改良碳青霉烯灭活试验(mCIM)检测碳青霉烯酶;利用MALDI-TOF MS技术筛选CRKp菌株特异性峰,采用MALDI-TOF MS进行同源性分析,多位点序列分型(MLST)方法作为验证。结果30株CRKp株mCIM均为阳性,其中24株含有共同特异性峰,特异性达到80%,符合区分阈值。蛋白质质量峰聚类分析显示,30株CRKp株分为三大簇,同时MLST结果表明,ST11/A型为27株、ST15/B型为2株,而ST2193/D型仅有1株。结论MALDI-TOF MS在CRKp鉴定中具有精准、快速的优点。MALDI-TOF MS能够有效进行CRKp同源性分析,并应用于院内感染流行暴发的监测。 Objective To screen specific peaks of carbapenem-resistant Klebsiella pneumonia(CRKp)and conduct homology analysis using matrix assisted laser desorption ionization time of flight mass spectrometry(MALDI-TOF MS).Methods Thirty strains of clinical CRKp isolates were collected.BioMérieux VITEK 2 Compact analyzer was used for bacterial identification and drug sensitivity test.Modified carbapenem inactivation test(mCIM)was used to detect carbapenemase.The specific peaks and homology analysis of CRKp strains were performed by using MALDI-TOF MS technology.Multilocus sequence typing(MLST)method was used for verification.Results All the 30 CRKp strains were positive to mCIM.Twenty-four of them contained a common specificity peak with a specificity of 80%,which met the discrimination threshold.Protein quality peak cluster analysis showed that the 30 CRKp strains fell into three clusters.And the MLST results showed that there were 27 strains of ST11/A type,2 strains of ST15/B type,and only 1 strain of ST2193/D.Conclusion MALDI-TOF MS has advantages in rapid and accurate identification of CRKp.It can also effectively perform homology analysis of CRKp and can be used to monitor the outbreak of nosocomial infection.
