摘要
基于中医病因病机研究,认为在肺小结节形成过程中肺气内虚无力或郁滞难行,乃肺小结节形成之初病变;气不顺则津停成痰,痰气胶着,日久可形成有形实邪;痰气不化,瘀血渐成,三者痹阻于脉络,可见肺小结节之貌或内部结构发生改变;邪气长居久积,渐生肺毒,与痰瘀搏结,耗伤正气,肺小结节恶化;人体正气亏虚为发病基础,贯穿于肺小结节发生发展的全过程。基于此,从气足不郁则疾不生、津行血运则化其形、除毒拔邪则防其恶的角度出发,对肺小结节的治疗进行探讨,可选用补肺汤、逍遥散、二陈汤、血府逐瘀汤等加减清热解毒药治疗。
Based on the research of etiology and pathogenesis of traditional Chinese medicine,it is considered that the deficiency of lung qi or stagnation of lung qi are the initial lesions of pulmonary small nodule formation.If qi is not smooth,body fluid will stop and transform into phlegm,and the phlegm and qi will stick together,which may form tangible excess pathogen over time.If phlegm and qi can’t be resolved,blood stasis will gradually develop.Then,phlegm,qi and blood stasis are blocked in the vessels,causing pulmonary small nodule and internal structure changes.Pathogenic qi accumulates for a long time,gradually produces lung toxin,accumulates with phlegm and blood stasis,consumes and injures healthy qi,and worsens pulmonary small nodule.Healthy qi deficiency is the basis of the disease,which runs through the whole process of the occurrence and development of pulmonary small nodule.Based on this,this paper explores the treatment of pulmonary small nodule from the perspective of sufficient qi without stagnation being healthy,circulation of body fluid and blood resolving tangible excess pathogen,removing toxin and expelling pathogen preventing deterioration.Bufei Decoction(补肺汤),Xiaoyao Powder(逍遥散),Erchen Decoction(二陈汤),and Xuefu Zhuyu Decoction(血府逐瘀汤)may be used in the treatment of pulmonary small nodule,combining with Chinese materia medica for clearing heat and removing toxin.
