摘要
目的探讨清肝敛疡汤治疗肝胃郁热型幽门螺杆菌(H.pylori)阳性消化性溃疡的疗效。方法选取2019年10月—2020年9月在北京中医药大学附属护国寺中医医院中医内科门诊就诊的肝胃郁热型H.pylori阳性活动期消化性溃疡患者74例为研究对象,随机分为对照组和试验组,每组37例。对照组予四联疗法治疗,试验组予清肝敛疡汤联合四联疗法治疗,2组疗程均为6周。比较2组患者治疗前后中医症状积分、H.pylori根除率、胃镜下溃疡愈合情况、临床总有效率及1年后溃疡和H.pylori复发率。结果试验组治疗后各单项中医症状积分及总积分均明显低于对照组,治疗后H.pylori根除率及总有效率均明显高于对照组,胃镜下溃疡愈合情况明显优于对照组,差异均有统计学意义(P均<0.05)。2组治疗过程中均未发生明显不良反应。试验组治疗1年后溃疡复发率和H.pylori复发率均明显低于对照组(P均<0.05)。结论清肝敛疡汤联合四联疗法治疗肝胃郁热型H.pylori阳性消化性溃疡疗效确切,不仅可以减轻患者的临床症状,提高H.pylori根除率和溃疡的愈合质量,还可以降低H.pylori和溃疡复发率,且安全性良好。
Objective It is to explore the curative effect of Qinggan Lianyang decoction in the treatment of Helicobacter pylori(H.pylori)positive peptic ulcer of type of liver and stomach stagnation heat.Methods A total of 74 patients with active H.pylori-positive peptic ulcer of type of liver-stomach stagnation heat who visited the outpatient department of traditional Chinese medicine of Huguosi Hospital of Traditional Chinese Medicine Affiliated to Beijing University of Chinese Medicine from October 2019 to September 2020 were selected as the research objects,and were randomly divided into control group and experimental group,each group had 37 cases.The control group was treated with quadruple therapy,while the experimental group was treated with Qinggan Lianyang decoction combined with quadruple therapy.The course of treatment in both groups was 6 weeks.The traditional Chinese medicine symptom scores before and after treatment,H.pylori eradication rate,ulcer healing under gastroscopy,clinical total effective rate,and ulcer and H.pylori recurrence rates after 1 year were compared between the two groups.Results After treatment,the scores of each symptom and total scores of the experimental group were significantly lower than those of the control group,the H.pylori eradication rate and total effective rate were significantly higher than those of the control group,the ulcer healing under gastroscopy was significantly better than that of the control group,the differences were all statistically significant(all P<0.05).There were no obvious adverse reactions during treatment in the two groups.The ulcer recurrence rate and H.pylori recurrence rate in the experimental group at 1 year after treatment were significantly lower than those in the control group(all P<0.05).Conclusion Qinggan Lianyang decoction combined with quadruple therapy has a definite curative effect in the treatment of H.pylori positive peptic ulcer of type of liver and stomach stagnation heat.It can not only relieve the clinical symptoms of patients,increase the
