摘要
目的观察达格列净对2型糖尿病胰岛素治疗后血糖控制不佳患者的治疗效果。方法抽取该院2019年6月—2021年1月收治的102例2型糖尿病患者为研究对象,以数字盲选法分为研究组和对照组,每组51例。对照组给予盐酸二甲双胍缓释片,研究组在此基础上给予达格列净治疗。对比两组患者的血糖水平、体质量、血压、24 h尿定量及每日胰岛素剂量变化情况、临床相关指标和不良反应发生率等。结果治疗后,研究组患者血糖水平低于对照组,差异有统计学意义(P<0.05);研究组体质量、血脂、血压、血尿酸及每日胰岛素剂量变化情况优于对照组,差异有统计学意义(P<0.05);研究组胰岛β细胞功能指数、胰岛素抵抗指数均高于对照组,差异有统计学意义(P<0.05);研究组肌酐、尿酸水平、丙氨酸转氨酶、谷草转氨酶水平、胆固醇、三酰甘油水平均低于对照组,差异有统计学意义(P<0.05);研究组不良反应发生率5.88%低于对照组的19.61%,差异有统计学意义(P<0.05)。结论将达格列净用于2型糖尿病胰岛素治疗后血糖控制效果差的患者,临床疗效尚佳,对其相关血糖指标有明显改善效果。
Objective To observe the therapeutic effect of Dapagliflozin on patients with type 2 diabetes after poor insulin control.Methods 102 patients with type 2 diabetes mellitus admitted to the hospital from June 2019 to January 2021 were selected and divided into study group and control group by digital blind selection.The control group was given metformin hydrochloride sustained release tablets,and the study group was given dapagliflozin treatment on this basis.The blood glucose level,body weight,blood pressure,24-hour urine quantification,daily insulin dose changes,clinically relevant indicators and incidence of adverse reactions were compared between the two groups.Results After treatment,the blood glucose level of the patients in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05);The changes of body weight,blood lipids,blood pressure,blood uric acid and daily insulin dose in the study group were better than those in the control group,and the difference was statistically significant(P<0.05);The isletβ-cell function index and insulin resistance index in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05);The levels of creatinine,uric acid,alanine aminotransferase,aspartate aminotransferase,cholesterol and triacylglycerol in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05);The incidence of adverse reactions in the study group was 5.88%lower than that in the control group,19.61%,and the difference was statistically significant(P<0.05).Conclusion Dapagliflozin has a good clinical effect in the treatment of type 2 diabetes mellitus with poor blood glucose control effect.
