The role of dietary intervention management mode in the patients with early diabetes nephropathy

摘要 目的:探索饮食干预管理模式在早期糖尿病肾病患者中的作用。方法:选取上海徐汇区枫林街道社区1~3期糖尿病肾病患者116例,按就诊顺序分为对照组及实验组各58例。对照组中有男性30例,女性28例,年龄为55~75岁,平均年龄为(64.5±9.5)岁;实验组中有男性32例,女性26例,年龄为56~74岁,平均年龄为(65.6±8.7)岁。根据早期糖尿病肾病患者的尿微量白蛋白/肌酐比值由营养师制定个性化的食谱,制定符合患者个体化教育路径等干预措施。比较干预前后两组患者的糖化血红蛋白、尿微量白蛋白/肌酐比值。结果:经过饮食干预6个月后,两组患者的糖化血红蛋白水平和尿微量白蛋白/肌酐比值均有下降,差异均有统计学意义(P<0.05),但实验组患者的糖化血红蛋白水平和尿微量白蛋白/肌酐比值低于对照组,差异均有统计学意义(P<0.05)。结论:对早期糖尿病肾病患者进行饮食干预对患者的病情有明显的控制作用。 Objective:To explore the effect of dietary intervention management mode in the patients with early diabetic nephropathy.Methods:One hundred and sixteen patients with stage 1~3 diabetes nephropathy in Fenglin Community of Xuhui District,Shanghai were selected,and according to the order of treatment,divided into a control group and an experimental group with 58 cases in each group.There were 30 males and 28 females in the control group,and the age ranged from 55 to 75 years old,with an average age of(64.5±9.5)years old;there were 32 males and 26 females in the experimental group,and the age ranged from 56 to 74 years old,with an average age of(65.6±8.7)years old.According to the urine microalbumin/creatinine ratio of patients with early diabetic nephropathy,a personalized diet was formulated by a nutritionist,and intervention measures such as individualized education path were formulated.The glycated hemoglobin and urinary microalbumin/creatinine ratios in the two groups were compared before and after the intervention.Results:After 6 months of dietary intervention,the levels of glycosylated hemoglobin and the ratio of urinary microalbumin/creatinine in the two groups were decreased,and the differences were statistically significant(P<0.05).However,the levels of glycosylated hemoglobin and the ratio of urinary microalbumin/creatinine in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Dietary intervention for patients with early diabetes nephropathy has a significant control effect on the patient’s condition.
