摘要
目的分析龙岩市武平县人间钩端螺旋体病(钩体病)流行状况和特征,为防控工作提供依据。方法采集健康人群血清,用钩体显微镜凝集试验进行检测,并分析菌群分布。结果武平县2017—2021年共报告钩体病10例,总体上呈下降趋势。共检测健康人群血清360份,阳性率21.1%(76/360),男、女阳性率差异无统计学意义;血清抗体阳性率在2018年高峰后下降;高年龄组(40~59岁)血清抗体阳性率为29.9%(44/147),而低年龄组(10~39岁)仅为15.0%(32/213,χ^(2)=11.61,P=0.01);农民的感染率为22.1%(64/290),学生的感染率为18.2%(12/66),差异无统计学意义(χ^(2)=0.484,P=0.487);2017—2018年主要菌群为秋季群、2019—2021年为澳洲群,存在主要菌群的变迁现象。结论武平县2017—2021年钩体病疫情和健康人群血清抗体阳性率总体上呈下降趋势,但菌群构成复杂,存在暴发的可能,仍应加强钩体病的防控工作。
Objective To analyze the epidemiological situation and characteristics of human leptospirosis in Wuping County from 2017to 2021for the disease control and prevention.Methods Serum samples of the residents were collected and detected by leptospira microscope agglutination test,and the bacterial flora distributions were analyzed.Results Totally ten cases of leptospirosis were reported from 2017to 2021,and the incidence generally showed decreasing tendency.Totally 360serum samples were detected and the positive rate was 21.1%(76/360)and no statistical significance in genders.Serum antibody positive rates decreased year by year after 2018.The positive rate of aged 4059was 29.9%(44/147),and was 15.0%(32/213)only for aged 1039(χ^(2)=11.61,P=0.01).The infection rates of farmers and students were 22.1%(64/290)and 18.2%(12/66)respectively,and no statistical significance.The main serum groups were Autumnalis from 2017to 2018and Australis from 2019to 2021.The main serum groups still were changing.Conclusion The leptospirosis cases and serum antibody positive rates of the residents in Wuping County showed decreasing tendency,but the bacterial composition of serotypes was complex and there existed a possibility of the disease outbreak.The control and prevention of leptospirosis in the county should be strengthened.
