2016—2020年某院血标本细菌耐药性监测

摘要 目的了解2016—2020年重庆市某土家族自治县人民医院临床血标本分离细菌的分布及耐药性。方法根据全国细菌耐药监测网方案,对临床血标本分离菌进行鉴定和药敏试验,按美国临床实验室标准化协会标准判断结果。结果血标本共分离出1478株非重复的细菌,其中革兰阳性菌552株(37.3%),革兰阴性菌926株(62.7%),其中最常见的细菌依次为大肠埃希菌、人葡萄球菌、肺炎克雷伯菌、表皮葡萄球菌和铜绿假单胞菌。耐甲氧西林金黄色葡萄球菌检出率为27.8%。耐碳青霉烯类抗菌药物的大肠埃希菌和肺炎克雷伯菌的检出率分别为0.64%和6.06%。铜绿假单胞菌对亚胺培南和美罗培南的耐药率均为10.5%,鲍曼不动杆菌对亚胺培南和美罗培南的耐药率分别33.3%和27.8%。结论该院血标本分离菌以革兰阴性菌为主,细菌耐药性整体低于全国和重庆水平,但耐碳青霉烯类肺炎克雷伯菌的检出率呈上升趋势,应加强血流感染病原菌的耐药性监测,促进抗菌药物的合理应用。 Objective To statistically analyze the bacterial distribution and drug resistance of blood culture during 2016 to 2020 in the People's Hospital of Tujia Autonomous County.Methods Bacterial identification and antimicrobial susceptibility tests were conducted according to the uniform technical program of China antimicrobial resistance surveillance system,antimicrobial susceptibility testing results were interpreted according to clinical and laboratory standards institute.Results A total of 1478 non-repeating bacteria were isolated from blood specimens,including 552 Gram-positive bacteria(37.3%)and 926 Gram-negative bacteria(62.7%).The most common bacteria in blood specimens were escherichia coli,staphylococcus hominis,klebsiella pneumoniae,staphylococcus epidermidis,and pseudomonas aeruginosa.The prevalence of methicillin-resistant strains was 27.8% in S.aureus.The prevalence of carbapenem-resistant escherichia coli and kebsiella pneumoniae were 0.64% and 6.06%,respectively.The drug resistance rates of pseudomonas aeruginosa to imipenem and meropenem were 10.5% and 10.5%,and those of acinetobacter baumannii to imipenem and meropenem were 33.3% and 27.8%,respectively.Conclusion Gram-negative bacteria are the main pathogens isolated from blood in the hospital.The prevalence of antibiotic-resistant isolates from blood culture in this hospital is generally lower than the national and Chongqing level,but isolation rate of carbapenem-resistant Klebsiella pneumoniae increased.It is necessary to strengthen surveillance on antimicrobial resistance of pathogens causing bloodstream infection and use antimicrobial agents rationally.
