摘要
目的:探究超声在卵巢卵泡膜细胞瘤诊断及鉴别诊断中的应用价值。方法:纳入2019年4月-2021年4月某院收治的88例疑似卵巢卵泡膜细胞瘤患者,所有患者均行彩色多普勒超声检查。以病理结果作为“金标准”,分析超声检查结果,对比超声与病理检出率;分析超声检查征象表现及手术病理表现;另分析超声在卵巢卵泡膜细胞瘤中的诊断价值。结果:经病理检查明确88例疑似患者中,共检出卵巢卵泡膜细胞瘤68例,囊实性肿瘤20例。超声检查明确卵巢卵泡膜细胞瘤74例,囊实性肿瘤12例,超声与病理检查在卵巢卵泡膜细胞瘤检出率比较中,无统计学差异(P<0.05);超声诊断卵巢卵泡膜细胞瘤灵敏度为97.06%(66/68)、特异度60.00%(12/20)、准确度分别为88.64%(78/88)、阳性预测值为89.19%(66/74)、阴性预测值为85.71%(12/14),Kappa检验显示:超声在卵巢卵泡膜细胞瘤诊断中与“金标准”的一致性尚可(Kappa值=0.638,P=0.000)。结论:卵巢卵泡膜细胞瘤在超声检查中具有操作简单、观察病灶清晰,在临床应用中开展更加普遍,但需注意与卵巢癌、卵巢囊肿等疾病进行鉴别,值得推广应用。
Objective:To explore the application value of ultrasonography in the diagnosis and differential diagnosis of ovarian theca cell tumor.Methods:A total of 88 patients with suspected ovarian theca cell tumor admitted to a hospital from April 2019 to April 2021 were included,and all patients underwent color Doppler ultrasonography.Taking the pathological results as the"gold standard",the results of ultrasonography were analyzed,and the detection rates of ultrasonography and pathology were compared;The manifestations of ultrasonography and surgical pathology were analyzed;The diagnostic value of ultrasonography in ovarian theca cell tumors was also analyzed.Results:Among the 88 suspected patients confirmed by pathological examination,68 cases of ovarian theca cell tumors were detected,and the remaining 20 cases were cystic and solid tumors.Ultrasound examination confirmed 74 cases of ovarian theca cell tumors,and the remaining 12 cases were cystic and solid tumors.There was no significant difference in the detection rate of ovarian theca cell tumors between ultrasonic and pathological examinations(P<0.05).The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of ultrasonography in the diagnosis of ovarian theca cell tumor were 97.06%(66/68),60.00%(12/20),88.64%(78/88),89.19%(66/74),and 85.71%(12/14),respectively.The Kappa test showed that the consistency of ultrasound with the"gold standard"in the diagnosis of ovarian theca cell tumor was acceptable(Kappa value=0.638,P=0.000).Conclusion:Ovarian theca cell tumor has the advantages of simple operation and clear lesions in ultrasound examination,and it is more common in clinical application.However,attention should be paid to the identification with ovarian cancer,ovarian cyst and other diseases,which is worthy of promotion and application.
