摘要
目的 研究老年患者脑白质高信号(White Matter Hyperintensity,WMH)严重程度与颈动脉内膜中层厚度(IMT)的相关性。方法 收集2019年12月至2021年3月国家康复辅具研究中心附属康复医院神经内科就诊的老年患者99例,经头颅MRI检查均存在WMH,所有患者完善颈动脉彩色超声检查(分为正常组、增厚组和斑块组)。通过Fezakas量表评价WMH评分。所有患者根据WMH评分分为1~3级。Pearson相关性分析脑白质高信号评分与颈动脉IMT的关系。结果 不同级别的WMH患者在年龄、高血压方面差异存在统计学意义(P<0.05,P<0.01);随颈动脉粥样硬化程度增加,WMH级别逐步提高,差异有统计学意义(χ^(2)=32.695,P=0.000);去除年龄及高血压因素,脑白质评分与颈动脉IMT分级呈正相关(r=0.272,P=0.007)。结论 老年患者WMH与颈动脉粥样硬化密切相关,颈动脉IMT可预测WMH的严重程度。
Objective To investigate the correlation between CIMT and WMH in elderly patients.Methods We retrospectively collected 99 elderly patients with who were treated in the Neurology department of Rehabilitation Hospital Affiliated to National Research Center for Rehabilitation Technical Aids from December 2019 to March 2021.All of them performed MRI and carotid ultrasound examination(divided into normal group,thickened group and plaque group).WMH scores were evaluated by the Fezakas scale.All patients were classified into grade 1 to 3 according to WMH score.Pearson correlation analysis of white matter high signal score and carotid artery IMT.Results There were statistically significant differences in age and hypertension among WMH patients of different grades(P<0.05,P<0.01).WMH grade increased with the increase of carotid atherosclerosis,and the difference was statistically significant(χ^(2)=32.695,P=0.000).Conclusion WMH is closely related to carotid atherosclerosis in elderly patients,and carotid IMT can predict the severity of WMH.
