摘要
目的 评价联合内镜下钛夹止血、兰索拉唑治疗老年上消化道出血的预后效果。方法 84例老年上消化道出血患者,以随机数字表法分为对照组及观察组,每组42例。对照组采取兰索拉唑治疗,观察组采取兰索拉唑联合内镜下钛夹止血治疗。比较两组患者的即时止血率、72 h止血率、并发症发生情况及治疗前后的凝血指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)]水平。结果 观察组患者即时止血率64.29%、72 h止血率97.62%均高于对照组的42.86%、69.05%,差异具有统计学意义(P<0.05)。两组患者并发症发生率比较差异无统计学意义(P>0.05)。治疗后,观察组患者的PT(12.60±0.90)s、APTT(29.01±2.15)s、FIB(3.22±0.40)g/L优于对照组的(14.60±0.85)s、(31.95±2.13)s、(2.83±0.45)g/L,差异具有统计学意义(P<0.05)。结论 内镜下钛夹止血与兰索拉唑联合治疗老年上消化道出血效果显著、安全性高,能促进患者凝血功能改善。
Objective To evaluate the prognostic effect of endoscopic titanium clip hemostasis combined with lansoprazole in the treatment of upper gastrointestinal bleeding in the elderly. Methods A total of 84 elderly patients with upper gastrointestinal bleeding were randomly divided into control group and observation group, with 42 cases in each group. The control group was treated with lansoprazole, and the observation group was treated with lansoprazole combined with endoscopic titanium clip hemostasis. Both groups were compared in terms of immediate hemostasis rate, 72-h hemostasis rate, occurrence of complications and the coagulation indexes [prothrombin time(PT), activated partial thromboplastin time(APTT), fibrinogen(FIB)] before and after treatment. Results The immediate hemostasis rate in the observation group was 64.29% and the 72-h hemostasis rate was 97.62%, which were higher than 42.86% and 69.05% in the control group, and the differences were statistically significant(P<0.05). There was no statistically significant difference in the complication rate between the two groups(P>0.05). After treatment, the PT(12.60±0.90) s, APTT(29.01±2.15) s, and FIB(3.22±0.40) g/L of the observation group were better than(14.60±0.85) s,(31.95±2.13) s, and(2.83±0.45) g/L of the control group, and the difference was statistically significant(P<0.05). Conclusion Endoscopic titanium clip hemostasis combined with lansoprazole has a significant effect and high safety in the treatment of elderly upper gastrointestinal bleeding, and can improve the coagulation function of patients.
