摘要
目的:探究评价急性非静脉曲张性上消化道出血治疗中内镜止血的应用效果。方法:研究对象选取时间为2019年3月~2020年2月,入选的78例急性非静脉曲张性上消化道出血患者均于本院就诊治疗。39例对照组患者止血方案为药物止血治疗,39例内镜组患者止血方案为内镜下金属钛夹止血治疗,分析和对比两组出血停止时间、输血量、肠鸣音恢复正常时间、住院时长、炎性因子水平、治疗效果和止血成功率、止血后后再出血情况以及并发症情况。结果:内镜组出血停止时间、肠鸣音恢复正常时间以及住院时长均短于对照组,输血量少于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后的肿瘤坏死因子-α、白细胞介素-4和C反应蛋白水平均明显降低,且内镜组患者治疗后各项指标水平明显低于对照组治疗后的水平,差异具有统计学意义(P<0.05)。内镜组患者的治疗总有效率明显高于对照组,止血成功率明显高于对照组,止血后的再出血率和并发症的发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论:内镜下金属钛夹止血治疗方案可以在更短的时间内有效帮助急性非静脉曲张性上消化道出血患者达成止血目的,有效缓解患者炎症反应,对患者身体恢复起到明显促进作用。
Objective:To explore and evaluate the application effect of endoscopic hemostasis in the treatment of acute non-variceal upper gastrointestinal bleeding.Methods:The research subjects were selected from March 2019 to February 2020,and 78 patients with acute non-variceal upper gastrointestinal bleeding were selected for treatment in our hospital.The hemostatic scheme of 39 patients in the traditional group was drug hemostasis,and the hemostatic scheme of 39 patients in the endoscopic group was endoscopic metal titanium clip hemostasis.The bleeding stop time,blood transfusion volume,recovery time of bowel sounds,length of hospital stay,level of inflammatory factors,treatment effect and rebleeding after successful hemostasis were analyzed and compared between the two groups.Results:The stopping time of bleeding,the recovery time of bowel sounds and the length of hospitalization in the endoscopic group were shorter than those in the traditional group,and the amount of blood transfusion was less than that in the traditional group(P<0.05).TNF after treatment in the two groups-α,the levels of IL-4 and CRP decreased significantly,and the levels of various indexes in the endoscopic group were significantly lower than those in the traditional group(P<0.05).The total effective rate of endoscopic group was significantly higher than that of traditional group,and the rebleeding rate after hemostasis was significantly lower than that of traditional group(P<0.05).Conclusion:Endoscopic metal titanium clip hemostasis treatment plan can effectively help patients with acute non-variceal upper gastrointestinal bleeding to achieve hemostasis in a shorter time,effectively relieve the inflammatory response of patients,and significantly promote the recovery of patients.
