摘要
目的 观察揿针对脑卒中后吞咽功能障碍的改善作用及对功能性磁共振成像(fMRI)的影响。方法 收集2019年3月~2021年3月浙江省人民医院收治的118例脑卒中后吞咽功能障碍患者,按照入院编号将其分为试验组与对照组,每组各59例,对照组予以吞咽功能训练,试验组在对照组基础上予以揿针埋针,连续治疗28 d。比较两组的临床疗效、不良反应及治疗前后的标准吞咽功能评估量表(SSA)评分、洼田饮水试验评分、f MRI测定结果及血清白蛋白(ALB)、总蛋白(TP)水平。结果 试验组总有效率显著高于对照组(P<0.05);治疗后,两组SSA评分和洼田饮水试验评分均较治疗前显著下降(P<0.05),两组ALB、TP较治疗前显著增加(P<0.05),试验组SSA评分和洼田饮水试验评分下降程度优于对照组(P<0.05),试验组ALB、TP增加程度优于对照组(P<0.05);治疗后,对照组额叶、顶叶、BA7区激活频率较治疗前有所增加,但差异无统计学意义(P>0.05),试验组额叶、顶叶、BA7区激活频率较治疗前显著增加(P<0.05);两组均无严重不良反应发生。结论 揿针对脑卒中后吞咽功能障碍的疗效较好,能改善患者吞咽功能,促进吞咽功能相关的脑区激活,提升患者营养状况,安全性高。
Objective To observe the effect of thumbtack needle on improvement of dysphagia after stroke and functional magnetic resonance imaging(fMRI). Methods A total of 118 patients with dysphagia after stroke admitted to Zhejiang Provincial People′s Hospital from March 2019 to March 2021 were selected. They were divided into the experimental group and the control group according to the admission number, with 59 patients in each group. The control group were given swallowing function training, and the experimental group were given additional thumbtack needle treatment for 28 days continuously. The clinical efficacy, adverse events, standard swallowing function assessment scale(SSA) scores, water swallow test scores, fMRI measurement results, serum albumin(ALB) and total protein(TP) levels before and after treatment were compared between the two groups. Results The total effective rate in the experimental group was significantly higher than that in the control group(P<0.05). The scores of SSA and water swallow test after treatment were significantly lower than those before treatment in both groups(P<0.05). The ALB and TP levels in both groups were significantly increased after treatment(P<0.05). The decreases of SSA and water swallow test scores in the experimental groups were better than those in the control group(P<0.05). The increases of ALB and TP levels in the experimental groups were better than those in the control group(P<0.05). After treatment, the activation frequencies of frontal lobe, parietal lobe, and BA7 area in the control group were increased than those before treatment, without statistically significant differences(P>0.05).After treatment,the activation frequencies of frontal lobe,parietal lobe, and BA7area in the experimental group were significantly higher than those before treatment(P<0.05). There were no serious adverse events in both groups. Conclusion Thumbtack needle has a good effect on dysphagia after stroke.It can improve swallowing function,promote activation of brain regions related to swal
