摘要
目的了解自然不孕妇女和健康体检的育龄妇女孕前TORCH感染状况。方法采用ELISA法对2018年12月至2019年11月期间郑州大学第一附属医院生殖医学中心进行医学干预受孕的自然不孕育龄妇女11597名和健康体检育龄妇女1547名进行TORCH特异性IgM和IgG抗体检测。结果自然不孕育龄妇女TORCH IgM抗体总阳性率为2.42%,健康体检育龄妇女为2.26%,差异无统计学意义(χ^(2)=0.15,P=0.70)。自然不孕和健康体检的育龄妇女中IgM抗体阳性率从高到低分别是弓形虫(Tox)-IgM(0.99%和0.90%)、风疹病毒(RV)-IgM(0.67%和0.90%)、单纯疱疹病毒(HSV)Ⅰ-IgM(0.34%和0.26%)、HSVⅡ-IgM(0.22%和0.13%)和巨细胞病毒(CMV)-IgM(0.20%和0.06%),其差异均无统计学意义;IgG抗体阳性率从高到低分别是CMV-IgG(96.20%和93.47%)、HSVⅠ-IgG(93.39%和91.40%)、RV-IgG(90.87%和88.49%)、HSVⅡ-IgG(14.26%和9.18%)和Tox-IgG(4.44%和4.40%),除Tox-IgG抗体外,自然不孕妇女其他病原体IgG抗体阳性率均高于孕前咨询健康体检育龄妇女,差异均有统计学意义(P<0.05)。在不同年龄组间,自然不孕和健康体检的育龄妇女中IgM抗体阳性率间差异均无统计学意义,而在IgG抗体方面,其>35岁组的Tox-、CMV-、HSVⅠ-和HSVⅡ-IgG抗体阳性率普遍高于20~35岁组,其中在健康体检育龄妇女>35岁组中HSV I-IgG抗体阳性率显著性高于20~35岁组(χ^(2)=4.15,P=0.042),在自然不孕育龄妇女>35岁组中HSVⅠ-和HSVⅡ-IgG抗体阳性率均显著性高于20~35岁组(χ^(2)=9.82,P=0.0017;χ^(2)=50.59,P<0.0001);但RV-IgG抗体阳性率均为20~35岁组显著性高于>35岁组(自然不孕组:χ^(2)=19.52,P<0.0001;健康体检组:χ^(2)=9.95,P=0.0016)。在不同季节间,仅自然不孕育龄妇女群体中Tox-和CMV-IgM抗体阳性率差异存在统计学意义,其中Tox-IgM抗体阳性率夏季和秋季中较高,CMV-IgM抗体阳性率仅在秋季中较高。结论本区域内自然不孕育龄妇女和健康体检育龄妇女中TORCH IgM抗体间无�
Objective To study the infection rate of Toxoplasma gondii(Tox),rubella virus(RV),cytomegalovirus(CMV),herpes simplex virusⅠ(HSVⅠ)and herpes simplex virusⅡ(HSVⅡ)(TORCH)among the natural infertile women and the women of childbearing age during pre-pregnant health examination.Methods Enzyme-linked immunosorbent assay(ELISA)was applied to detect anti-TORCH-IgM and-IgG in the plasma samples of 11597 natural infertile women and 1547 women of childbearing age during pre-pregnant health examination from December 2018 to November 2019.Results The total positive rate of anti-TORCH-IgM was 2.42%and 2.26%in the natural infertile women and the women of childbearing age during pre-pregnant health examination,respectively,and no statistical difference was found between the 2 groups(χ^(2)=0.15,P=0.70).The highest positive rate was anti-Tox-IgM(0.99%and 0.90%),and then in turn were anti-RV-IgM(0.67%and 0.90%),anti-HSVⅠ-IgM(0.34%and 0.26%),anti-HSVⅡ-IgM(0.22%and 0.13%)and anti-CMV-IgM(0.20%and 0.06%)in the two groups.No statistical difference was shown between the natural infertile women and the women of childbearing age during pre-pregnant health examination.Among the various anti-TORCH-IgG antibodies,the highest positive rate was anti-CMV-IgG(96.20%and 93.47%),then in turn were anti-HSVⅠ-IgG(93.39%and 91.40%)and anti-RV-IgG(90.87%and 88.49%),anti-HSVⅡ-IgG(14.26%and 9.18%)and anti-Tox-IgG(4.44%and 4.40%).Except for Tox-IgG antibody,the positive rates of IgG antibodies of other pathogens in natural infertile women were all higher than those in the women of childbearing age during pre-pregnant health examination with statistical significance(P<0.05).Among different age groups,there was no significant difference for the positive rate of IgM antibody between the natural infertile women and the women of childbearing age during health examination,but the positive rates of Tox-,CMV-,HSVⅠ-and HSVⅡ-IgG antibodies in the>35-year-old group were generally higher than those in the 20 to 35-year-old group.The positive rat
