摘要
目的探讨阴道内环境与辅助生殖早期妊娠丢失的相关性,为本病防治提供可靠临床依据。方法选择2013-04/2021-04月于作者医院生殖中心行辅助生殖治疗后早期妊娠丢失患者184例作为观察组,选取同期行辅助生殖治疗后正常分娩患者196例作为对照组。观察两组患者阴道分泌物、生殖道解脲支原体(Ureaplasma urealyticum,UU)和人型支原体(Mycoplasma hominis,MH)阳性检出率、转阴率与辅助生殖早期妊娠丢失的相关性。结果观察组UU、MH、UU+MH的阳性检出率高于对照组,且差异有统计学意义(P<0.05);治疗后两组患者转阴率比较,差异无统计学意义(P>0.05);两组患者阴道分泌物检验结果比较,差异无统计学意义(P>0.05)。Logistic回归分析显示,辅助生殖早期妊娠丢失与UU、MH感染无明显相关性,差异无统计学意义(P>0.05)。结论辅助生殖早期妊娠丢失患者UU、MH、UU+MH感染率明显高于正常分娩患者;经治疗支原体转阴不能改变早期妊娠丢失结局;阴道分泌物异常可能不是辅助生殖治疗后早期妊娠丢失的原因;UU、MH不是辅助生殖早期妊娠丢失的独立危险因素。
Objective To explore the correlation between vaginal environment and early pregnancy loss in assisted reproduction,and to provide reliable clinical basis for the prevention and treatment of this disease.Methods A total of 184 cases of early pregnancy loss patients who received assisted reproduction therapy in the reproductive center of author′s hospital from April 2013 to April 2021 were selected as the observation group,and 196 cases of normal delivery patients who received assisted reproduction therapy during the same period were selected as the control group.The correlation between positive rate,negative conversion rates of Ureaplasma urealyticum(UU),Mycoplasma hominis(MH)in vaginal secretion and early pregnancy loss in assisted reproduction in the two groups were observed.Results The positive rates of UU,MH and UU+MH in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).After treatments,there was no significant difference in the negative conversion rate between two groups(P>0.05).There was no significant difference in vaginal secretion test results between the two groups(P>0.05).Logistic analysis showed that there was no significant correlation between early pregnancy loss and infection of UU and MH(P>0.05).Conclusion The infection rates of UU,MH and UU+MH in early pregnancy loss patients with assisted reproduction are significantly higher than those in normal delivery patients.Treatments of mycoplasma negative can not change the outcome of early pregnancy loss;abnormal vaginal discharge may not be the cause of early pregnancy loss after assisted reproduction therapy.Neither UU nor MH are independent risk factors for early pregnancy loss during assisted reproduction.
