摘要
目的:探讨葛酮通络胶囊联合沙库巴曲缬沙坦钠治疗扩张型心肌病心力衰竭(DCM-CHF)患者的效果及对血清肿瘤坏死因子α刺激基因-6(TSG-6)、骨膜蛋白(POSTN)、转化生长因子-β1(TGF-β1)水平的影响。方法:选取2019年1月-2021年1月佳木斯市妇幼保健院收治的70例DCM-CHF患者,按照随机数字表法将其分为对照组和观察组,每组35例。对照组采用沙库巴曲缬沙坦钠治疗,观察组在对照组的基础上加用葛酮通络胶囊。比较两组疗效、心功能[左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左室射血分数(LVEF)]、TSG-6、POSTN、TGF-β1水平及生活质量评分。结果:观察组总有效率为88.57%,高于对照组的62.86%,差异有统计学意义(P<0.05)。治疗后,两组LVEDD、LVESD、血清TSG-6、POSTN、TGF-β1、症状、情绪状态、人际关系、娱乐活动、饮食睡眠评分均低于治疗前,LVEF均高于治疗前,且观察组LVEDD、LVESD、血清TSG-6、POSTN、TGF-β1、症状、情绪状态、人际关系、娱乐活动、饮食睡眠评分均低于对照组,LVEF高于对照组,差异均有统计学意义(P<0.05)。结论:葛酮通络胶囊联合沙库巴曲缬沙坦钠治疗DCM-CHF能够提高效果,逆转心室重构,改善心功能及生活质量,其机制可能与降低血清TSG-6、POSTN、TGF-β1水平有关。
Objective:To investigate the effect of Getong Tongluo Capsules combined with Sarcobactrum Valsartan Sodium in the treatment of dilated cardiomyopathy-chronic heart failure(DCM-CHF)and its influence on the serum levels of tumor necrosis factorαstimulating gene-6(TSG-6),periostin(POSTN)and transforming growth factor-β1(TGF-β1)in patients.Method:A total of 70 DCM-CHF patients admitted to Jiamusi Maternal and Child Health Hospital from January 2019 to January 2021 were selected,they were divided into control group and observation group according to random number table method,35 cases in each group.The control group was treated with Sarcobactrum Valsartan Sodium,and the observation group was treated with Getong Tongluo Capsules on the basis of the control group.Efficacy,cardiac function[left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF)],TSG-6,POSTN,TGF-β1 and quality of life score were compared between two groups.Result:The total effective rate of the observation group was 88.57%,which was higher than 62.86%of the control group,the difference was statistically significant(P<0.05).After treatment,LVEDD,LVESD,TSG-6,POSTN,TGF-β1 levels,symptoms,emotional state,interpersonal relationship,recreational activities,diet and sleep scores in both groups were lower than those before treatment,while LVEF in both groups were higher than those before treatment,LVEDD,LVESD,TSG-6,POSTN,TGF-β1,symptoms,emotional state,interpersonal relationship,recreational activities,diet and sleep scores in the observation group were lower than those in the control group,while LVEF in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Getong Tongluo Capsules combined with Sarcobactrum Valsartan Sodium can improve the curative effect of DCM-CHF,reverse ventricular remodeling,improve cardiac function and quality of life,the mechanism may be related to the reduction of serum TSG-6,POSTN
