摘要
目的观察柴芩和胃汤治疗慢性非萎缩性胃炎肝胃不和证的临床疗效。方法选择2018年1月至2021年1月安徽省庐江县中医院内七科慢性非萎缩性胃炎肝胃不和证门诊患者98例,按照随机数字表法分为对照组和观察组,各49例。对照组采用奥美拉唑、铝碳酸镁、枸橼酸莫沙必利等常规治疗,观察组在常规治疗基础上联合柴芩和胃汤治疗,2组均治疗1个月。比较2组患者治疗前后中医证候评分、血清炎症指标水平、胃镜观察指标,治疗前及治疗后3个月评价临床疗效及幽门螺杆菌(Hp)转阴率。结果2组治疗后中医证候各项评分均低于本组治疗前(P<0.05),且观察组治疗后中医证候各项评分均低于对照组治疗后(P<0.05)。2组治疗后血清炎症指标血清超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)水平均较本组治疗前降低(P<0.05),且观察组治疗后均低于对照组治疗后(P<0.05)。观察组总有效率89.79%(44/49),对照组总有效率73.47%(36/49),观察组疗效优于对照组(P<0.05)。观察组Hp转阴率87.76%(43/49),对照组Hp转阴率69.39%(34/49),观察组Hp转阴率高于对照组(P<0.05)。治疗后3个月2组胃镜观察指标糜烂、花斑状红白相间、充血水肿评分均较本组治疗前降低(P<0.05),且观察组治疗后3个月均低于对照组治疗后3个月(P<0.05)。结论柴芩和胃汤能够减轻慢性非萎缩性胃炎肝胃不和证中医证候及胃黏膜病变,抑制炎性反应,提高Hp转阴率。
Objective To observe the curative effect of Chaiqin Hewei Decoction on chronic non-atrophic gastritis(CNAG)(liver stomach disharmony syndrome).Methods Totally 98 CNAG patients(liver stomach disharmony syndrome)in Outpatient of Seven Departments of Internal Medicine,Lujiang County Hospital of TCM from January 2018 to January 2021 were randomly divided into the control group(n=49)and the observation group(n=49).All patients were managed by conventional therapy(omeprazole,aluminum magnesium carbonate,mosapride citrate),and those in the observation group were additionally treated with Chaiqin Hewei Decoction.Treated for 1 month,aiming to compare traditional Chinese medicine(TCM)syndrome score,gastroscopy observation indexes,serum inflammatory cytokines;the comprehensive clinical effect and the negative rate of Helicobacter pylori(Hp)were assessed.Results After treatment,TCM symptom scores in groups were decreased(P<0.05),which were decreased in the observation group compared with the control group(P<0.05);serum inflammatory factors(serum high sensitivity C reactive protein[hs-CRP],interleukin-6[IL-6],tumor necrosis factor-α[TNF-α])in groups were decreased(P<0.05),the reduction in the observation group was more prevalent(P<0.05).The overall effective rate in the observation group was better than that in the control group(89.79%[44/49]vs 73.47%[36/49],[P<0.05],respectively);the negative rate of Hp was 87.76%(43/49)in the observation group,higher than that of 69.39%(34/49)in the control group(P<0.05).The gastroscopic observation indexes(erosion,flower patches with red-white interphase,congestion and edema)in groups were decreased(P<0.05),which were decreased(P<0.05)in the observation group relevant to the control group(P<0.05).Conclusion For CNAG patients(liver stomach disharmony syndrome),Chaiqin Hewei Decoction can reduce TCM syndromes and gastric mucosal lesions,inhibit inflammatory factors,and increase the negative rate of Hp.
