摘要
目的观察瘿瘤散结方治疗良性甲状腺结节的临床疗效及安全性。方法将80例良性甲状腺结节患者按照随机数字表法分为2组。对照组40例予小剂量左甲状腺素钠片治疗,治疗组40例予瘿瘤散结方治疗,2组均连续治疗2个月后统计临床疗效。比较2组治疗前后中医证候积分、最大结节的最大直径变化。检测2组治疗前后甲状腺功能指标促甲状腺激素(TSH)、三碘甲状腺原氨酸(T_(3))、甲状腺素(T_(4))、游离三碘甲状腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))和肝肾功能指标天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、尿素氮(BUN)、肌酐(Cr)变化情况,并详细记录治疗期间2组相关不良反应发生情况。结果治疗后2组中医证候各项评分及总分均较本组治疗前降低(P<0.05),且治疗组低于对照组(P<0.05)。治疗后2组最大结节的最大直径均较本组治疗前缩小(P<0.05),且治疗组小于对照组(P<0.05)。治疗组总有效率87.5%(35/40),对照组总有效率57.5%(23/40),2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。2组治疗前后甲状腺功能、肝肾功能指标无明显变化(P>0.05),组间比较差异也均无统计学意义(P>0.05),治疗期间均无不良反应。结论瘿瘤散结方治疗良性甲状腺结节的效果优于小剂量左甲状腺素钠片,且临床使用安全。
Objective To observe the clinical efficacy and safety of Yingliu Sanjie Formula for benign thyroid nodules(BTNs).Methods A total of 80 patients with BTNs were randomly divided into the treatment group and the control group,with 40 cases in each group.The control group was treated with low-dose of levothyroxine sodium tablets,and the treatment group with Yingliu Sanjie Formula.Treated for 2 months,aiming to compare traditional chinese medicine(TCM)syndrome score,the maximum diameter of the largest thyroid nodule,thyroid function indexes(thyroid-stimulating hormone[TSH],triiodothyronine[T_(3)],thyroxine[T_(4)],free triiodothyronine[FT_(3)],free thyroid hormone[FT_(4)]),liver and kidney function indexes(aspartate aminotransferase[AST],alanine aminotransferase[ALT],blood urine nitrogen[BUN],creatinine[Cr]);the curative effect and adverse reaction were assessed.Results After treatment,TCM symptom scores in groups were decreased(P<0.05),which were decreased in the treatment group compared with the control group(P<0.05);the maximum diameter of the largest thyroid nodule in groups was decreased(P<0.05),the improvement of which in treatment group was significantly better(P<0.05).The overall effective rate was 87.5%(35/40)in the treatment group,higher than that of 57.5%(23/40)in the control group,the difference of two groups being significant(P<0.05).There were no significant changes in thyroid function,liver and kidney function before and after treatment in groups(P>0.05),and with no adverse reactions.Conclusion For patients with BTNs,the effect of Yingliu Sanjie Formula is superior to low-dose of levothyroxine sodium tablets,which is safe for clinical use.
