摘要
目的 探讨吻合皮神经的腓肠神经营养血管皮瓣修复足踝部皮肤软组织缺损的临床效果。方法 70例足踝部皮肤软组织缺损患者,采取随机数字表法分为对照组和观察组,各35例。对照组采取负压封闭引流术(VSD),观察组采取吻合皮神经的腓肠神经营养血管皮瓣修复术。对比两组患者的满意度,生活质量,临床疗效,不良事件发生率。结果 观察组患者的满意度94.29%高于对照组的71.43%,差异具有统计学意义(P<0.05)。观察组患者的社会功能、心理影响、疾病影响评分分别为(7.61±0.24)、(8.52±1.02)、(8.69±0.34)分,均高于对照组的(6.84±1.37)、(7.65±1.21)、(7.85±1.58)分,差异具有统计学意义(P<0.05)。观察组患者的临床总有效率91.43%高于对照组的71.43%,差异具有统计学意义(P<0.05)。观察组患者的不良事件发生率11.43%低于对照组的34.29%,差异具有统计学意义(P<0.05)。结论 针对足踝部皮肤软组织缺损采取吻合皮神经的腓肠神经营养血管皮瓣修复临床疗效显著,值得推广。
Objective To discuss the clinical effect of sural neurovascular flap of anastomotic cutaneous nerve to repair the skin and soft tissue defects of the foot and ankle. Methods A total of 70 patients with skin and soft tissue defects of the foot and ankle were divided into control group and observation group according to random numerical table, with 35 cases in each group. The control group received vacuum sealing drainage(VSD), while the observation group received sural neurovascular flap repair with anastomotic cutaneous nerve. The satisfaction rate, quality of life, clinical eficacy, and occurrence of adverse events were compared between the two groups.Results The satisfaction rate 94.29% in the observation group was higher than 71.43% in the control group, and the difference was statistically significant(P<0.05). The scores of social function, psychological impact and disease impact in the observation group were(7.61±0.24),(8.52±1.02), and(8.69±0.34) points, which were higher than(6.84±1.37),(7.65±1.21), and(7.85±1.58) points in the control group, and the difference was statistically significant(P<0.05). The total clinical effective rate 91.43% of the observation group was higher than 71.43% of the control group, and the difference was statistically significant(P<0.05). The incidence of adverse events 11.43%in the observation group was lower than 34.29% in the control group, and the difference was statistically significant(P<0.05). Conclusion Sural neurovascular flap of anastomotic cutaneous nerve has significant clinical efficacy to repair the skin and soft tissue defects of the foot and ankle, and is worthy of promotion.
