摘要
目的:探讨高血压脑出血患者肺部感染血清炎性指标与和肽素的变化及意义。方法:选择高血压脑出血合并肺部感染患者72例设为联合组,同期选择单纯高血压脑出血患者72例设为对照组。所有患者均给予和肽素、降钙素原(PCT)、白细胞介素-8(IL-8)检测,随访调查患者的预后并进行相关性分析。结果:联合组患者血清和肽素、PCT、IL-8含量高于对照组,差异有统计学意义(P<0.05)。在144例患者中,Spearsman分析显示肺部感染与和肽素、PCT、IL-8含量存在相关性(P<0.05)。随访1年,联合组的死亡率16.7%高于对照组的4.2%,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示和肽素、PCT、IL-8为影响患者预后死亡的主要危险因素(P<0.05)。结论:高血压脑出血合并肺部感染患者多伴有血清和肽素、PCT、IL-8的高表达,与肺部感染的发生存在相关性,也是导致患者预后死亡的主要危险因素。
Objective To investigate the changes and significance of serum inflammatory indexes and copeptin in patients with hypertensive intracerebral hemorrhage.Method 72 cases of patients with hypertensive cerebral hemorrhage and pulmonary infection were selected as the combined group,and 72 cases of patients with isolated hypertensive cerebral hemorrhage were selected as the control group during the same period.All patients were detected with copeptin,procalcitonin(PCT),and interleukin(IL)-8,followed up the prognosis of the patients and given correlation analysis.Results The levels of serum and copeptin,PCT and IL-8 in the combined group were higher than those in the control group(P<0.05).In the 144 patients,Spearsman analysis showed that lung infection were correlated with copeptin,PCT,and IL-8 content(P<0.05).After 1 year of followed-up,the mortality rate of the combined group were 16.7%,which were higher than 4.2%of the control group(P<0.05).Multivariate logistic regression analysis showed that copeptin,PCT,and IL-8 were the main risk factors affected the prognosis of death(P<0.05).Conclusion Patients with hypertensive intracerebral hemorrhage and pulmonary infection are mostly accompanied by high expressions of serum copeptin,PCT,and IL-8,which are related to the occurrence of pulmonary infection and are also the main risk factors leading to the prognosis and death of patients.
