摘要
目的:探索声脉冲辐射力弹性成像技术(ARFI)联合血清磷脂酰肌醇蛋白多糖3(GPC3)对胆道闭锁患儿的诊断及其与肝纤维化程度的相关性。方法:回顾性分析2020年4月-2022年2月的74例正常体检的健康小儿(对照组)、76例经皮肝穿刺活检病理证实为胆道闭锁患儿(观察组)的临床资料,均进行血清GPC3、ARFI检测,Spearman法分析肝纤维分级与肝剪切波传播速度(SWV)、GPC3相关性,ROC曲线分析各方式的诊断效能。结果:观察组SWV、血清GPC3水平均高于对照组(P<0.05),F3、F4级SWV、血清GPC3水平均高于F1、F2级(P<0.05)。经Spearman分析,肝纤维分级与SWV、GPC3均呈正相关性(r_(s)=0.766、0.805,P<0.001);经ROC曲线分析,SWV、GPC3及两项联合诊断胆道闭锁的AUC值分别为0.776、0.828、0.926,SWV、GPC3及两项联合诊断胆道闭锁肝纤维F4级的AUC值分别为0.831、0.810、0.957。结论:GPC3、ARFI诊断胆道闭锁患儿效能较高,且表达量与胆道闭锁的肝纤维化分级呈正相关性。
Objective:To explore the diagnosis of acoustic radiation force impulse imaging(ARFI)combined with serum phosphatidylinositol proteoglycan 3(GPC3)in children with biliary atresia and its correlation with the degree of liver fibrosis.Method:The clinical data of 74 healthy children with normal physical examination(the control group)and 76 children with biliary atresia confirmed by percutaneous liver biopsy(the observation group)from April 2020 to February 2022 were analyzed retrospectively,serum GPC3 and ARFI were detected,and Spearman method was used to analyze the correlation between liver fiber grade and hepatic shear wave velocity value(SWV)and GPC3,ROC curve was used to analyze the efficacy of each diagnostic method.Result:The SWV and serum GPC3 level in the observation group were higher than those in the control group(P<0.05),the SWV and serum GPC3 level of F3,F4 were higher than those of F1,F2(P<0.05).Spearman showed that there were positive correlation between liver fiber grade and SWV and GPC3(r_(s)=0.766,0.805,P<0.001);through ROC curve analysis,the AUC values of SWV,GPC3 and their combination in the diagnosis of biliary atresia were 0.776,0.828 and 0.926 respectively,the AUC values of SWV,GPC3 and their combination in the diagnosis of biliary atresia were 0.831,0.810 and 0.957 respectively.Conclusion:GPC3 and ARFI are effective in the diagnosis of biliary atresia,and the expression is positively correlated with the grade of hepatic fibrosis of biliary atresia.
