摘要
目的观察马应龙麝香痔疮膏联合地奥司明片治疗肛肠疾病患者术后水肿的临床效果及对视觉模拟评分法(VAS)评分的影响。方法选取2019年6月—2020年6月松滋市人民医院收治的肛肠疾病患者100例,根据住院单双号随机分为联合组和地奥司明组,每组50例。2组患者均行手术治疗,术后地奥司明组给予地奥司明片治疗,联合组在地奥司明组基础上联合马应龙麝香痔疮膏治疗,2组均连续治疗5 d。比较2组治疗前后肛周疼痛VAS评分、水肿评分及机体应激指标去甲肾上腺素、血清皮质醇水平,治疗总有效率、肛肠疾病术后继发感染率。结果治疗5 d后,2组肛周疼痛VAS评分、水肿评分均较治疗前降低,2组去甲肾上腺素、血清皮质醇水平均较治疗前下降,且联合组低于地奥司明组(P均<0.01);联合组治疗总有效率为100.00%,高于地奥司明组的80.00%(χ^(2)=11.111,P=0.001);联合组肛肠疾病术后继发感染1例,地奥司明组发生12例,联合组术后继发感染发生率为2.00%,低于地奥司明组的24.00%(χ^(2)=10.698,P=0.001)。结论肛肠疾病术后患者采用马应龙麝香痔疮膏联合地奥司明片治疗效果确切,可减轻疼痛和加速疼痛消失,减轻对患者的不良应激,并可有效提高治疗总有效率,是一种有效的治疗模式,值得推广应用。
Objective To investigate the effect of Mayinglong musk hemorrhoids ointment combined with diosmin tablets on postoperative edema of anorectal diseases and its influence on VAS score. Methods A total of 100 patients with anorectal disease admitted to Songzi People’s Hospital from June 2019 to June 2020 were selected and randomly divided into a combined group and a diosmin group according to their hospitalization numbers,with 50 cases in each group. Both groups were treated with surgery,after operation diosmin group was treated with diosmin tablets,and the combined group was treated with Mayinglong musk hemorrhoids ointment combined with diosmin tablets. Both groups were treated for 5 days. VAS score,edema score,noradrenaline,serum cortisol,total effective rate and postoperative infection rate of anorectal diseases were compared before and after treatment. Results After 5 days of treatment,the VAS score and edema score of perianal pain in both groups were lower than those before treatment,the levels of noradrenaline and serum cortisol in both groups were lower than those before treatment,and the combined group was lower than the diosmin group (all P < 0. 01). The total effective rate in the combined group was 100. 00%,which was higher than that in the diosmin group (80. 00%),the difference was statistically significant (χ^(2)= 11. 111,P = 0. 001). There was 1 case of postoperative secondary infection of anorectal disease in the combined group and 12 cases in the diosmin group. The incidence of postoperative secondary infection in the combined group was 2. 00%,which was lower than 24. 00% in the diosmin group (χ^(2)= 10. 698,P = 0. 001). Conclusion The combination of Mayinglong musk hemorrhoids ointment and diosmin tablets is effective in the treatment of anal and intestinal diseases. It can reduce pain and accelerate the disappearance of pain,reduce the adverse stress to patients,and effectively improve the total effective level of patients.
