摘要
目的:探讨超声评估妊娠期附件扭转声像图特点及临床价值。方法:回顾性分析2015年1月—2020年2月于北京市朝阳区妇幼保健院经超声诊断的孕期附件扭转患者25例,以术后病理诊断结果为金标准,计算诊断符合率。结果:25例超声诊断附件扭转的病例中,21例术后诊断为附件扭转,符合率84%,误诊率16%;附件扭转≥360°者15例,13例附件坏死行切除术,坏死率约87%,2例扭转270°,4例扭转180°,均未坏死;发病3天内就诊者7例,坏死2例,坏死率28%;发病1~2周者14例,坏死12例,坏死率85%;手术及病理结果:7例为卵巢畸胎瘤扭转,13侧附件单纯囊肿扭转,1例积水输卵管扭转。结论:根据附件区特异性超声影像特征,结合临床症状超声诊断附件扭转准确率高。能够为疾病诊断提供重要条件,也适合临床上的广泛运用和推广。
Objective To explore the characteristics and clinical value of adnexal torsion during pregnancy.Methods Twenty five patients with adnexal torsion during pregnancy diagnosed by ultrasound in Beijing Chaoyang District Maternal and child health hospital from January 2015 to February 2020 were analyzed retrospectively.The diagnostic coincidence rate was calculated based on the postoperative pathological diagnosis results as the gold standard.Results Among the 25 cases,21 were diagnosed with ultrasound as adnexal torsion,which the accuracy was 84%,the coincidence was 16%,there were 15 cases with adnexal torsion≥360°,13 with necrosis,about 87%;2 cases with adnexal torsion 270°,4 cases with adnexal torsion 180°,no necrosis;Among the 7 patients who went to the doctor within 3 days of the onset,2 patients had necrosis,and the necrosis rate was 28%;14 patients had necrosis within 1 to 2 weeks,and 12 patients had necrosis,and the necrosis rate was 85%;Surgical and pathological results:7 cases were ovarian teratoma torsion,13 adnexal simple cyst torsion,and 1 hydrosalpinx torsion.Conclusion According to the characteristics of ultrasound area and the torsion accuracy of clinical symptoms.It is suitable for widespread clinical application and promotion.
