摘要
目的分析人类疱疹病毒第四型(EB)病毒所致传染性单核细胞增多症患儿病毒脱氧核糖核酸峰值载量(EBV-DNA)与肝功能的相关性。方法选取江西省儿童医院2020年1月至2021年12月收治的60例EB病毒所致传染性单核细胞增多症患儿作为观察组,另选取60例普通发热或呼吸道感染患儿为对照组。比较两组患儿血清白细胞介素-6(IL-6)、EBV-DNA、CD3^(+)及肝功能指标,采用logistic回归模型分析发生EB病毒所致传染性单核细胞增多症的影响因素,EBV-DNA、IL-6、CD3^(+)与肝功能的相关性采用Pearson相关性分析。结果观察组IL-6、EBV-DNA水平及血清丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、总胆固醇(TC)高于对照组,CD3^(+)低于对照组,差异有统计学意义(P<0.05)。logistic回归模型结果显示,IL-6(OR=1.063,95%CI=2.899~1.673)、EBV-DNA(OR=1.079,95%CI=2.983~1.257)升高是发生EB病毒所致传染性单核细胞增多症的独立危险因素,CD3^(+)(OR=0.512,95%CI=0.036~0.646)是发生EB病毒所致传染性单核细胞增多症的保护因素(P<0.05)。Pearson相关性分析显示,EBV-DNA、IL-6与ALT、AST、TC均成正相关(r>0,P<0.05),CD3^(+)与ALT、AST、TC均成负相关(r<0,P<0.05)。结论CD3^(+)、IL-6、EBV-DNA是EB病毒所致传染性单核细胞增多症的独立影响因素,且与肝功能密切相关。
Objective To analyze the correlation between peak load of Epstein-Barr virus deoxyribonucleic acid(EBV-DNA)and liver function in children with infectious mononucleosis induced by Epstein-Barr virus(EB virus).Methods A total of 60 children with EB virus-induced infectious mononucleosis who were admitted to Jiangxi Children′s Hospital from January 2020 to December 2021 were selected as the observation group.Meanwhile,60 children with common fever or respiratory tract infection were selected as the control group.Serum interleukin-6(IL-6),EBV-DNA,CD3^(+)and liver function indexes were compared between the two groups.Logistic regression model was used to analyze the influencing factors of infectious mononucleosis caused by EB virus.Pearson correlation analysis was used to analyze the correlation between EBV-DNA,IL-6,CD3^(+)and liver function.Results The levels of IL-6,EBV-DNA,serum alanine aminotransferase(ALT),aspartate aminotransferase(AST)and total cholesterol(TC)in the observation group were higher than those in the control group,and CD3^(+)was lower than that in the control group,the differences were statistically significant(P<0.05).The results of logistic regression model showed that the increase of IL-6(OR=1.063,95%CI=2.899-1.673)and EBV-DNA(OR=1.079,95%CI=2.983-1.257)was the risk factors of infectious mononucleosis caused by EB virus,and CD3^(+)(OR=0.512,95%CI=0.036-0.646)was the protective factor of infectious mononucleosis caused by EB virus(P<0.05).Pearson correlation analysis showed that EBV-DNA and IL-6 were positively correlated with ALT,AST and TC(r>0,P<0.05),and CD3^(+)was negatively correlated with ALT,AST and TC(r<0,P<0.05).Conclusion CD3^(+),IL-6 and EBV-DNA are independent influencing factors of infectious mononucleosis caused by EB virus,and are closely related to liver function.
