摘要
目的:探讨基于循证案例的预警干预在先天性肠闭锁患儿术后护理中的应用效果。方法:将2018年6月1日~2020年6月30日收治的64例先天性肠闭锁患儿随机分为预警组和对照组各32例,对照组采用常规肠闭锁术后护理措施,预警组在对照组基础上实施基于循证案例的预警干预。比较两组干预效果。结果:1个月后,预警组并发症总发生率低于对照组(P<0.05);预警组首次排气时间、排便时间及肠鸣音恢复时间均短于对照组(P<0.01);护理第7、14天,预警组吸吮和吞咽能力评分高于护理前(P<0.01),且高于同期对照组(P<0.01,P<0.05);预警组住院时间短于对照组(P<0.01)、二次手术例数少于对照组(P<0.05)。结论:基于循证案例的预警干预可改善先天性肠闭锁患儿胃肠功能恢复情况,降低术后并发症发生率,恢复患儿吸吮和吞咽能力,减少患儿二次手术情况。
Objective:To explore the application effect of early warning intervention based on evidence-based cases on postoperative nursing in children with congenital intestinal atresia.Methods:A total of 64 children with congenital intestinal atresia admitted during June 1,2018 to June 30,2020 were randomly divided into the early warning group and the control group each with 32 cases.The control group was given conventional postoperative nursing for intestinal atresia,and the early warning group was given early warning nursing of evidence-based cases on the basis of the control group.Intervention effect was compared between the two groups.Results:After 1 month of intervention,the total incidence rate of complications was lower in the early warning groip than that in the control group(P<0.05).The first exhaust time,defecation time and bowel sound recovery time were shorter in the early warning group than those in the control group(P<0.01).The scores of sucking and swallowing ability in the early warning group on the 7th d and 14th d after nursing were higher than those before nursing(P<0.01),and higher than those in the control group iat the same time(P<0.01,P<0.05).The hospital stay was shorter in the early warning group than that in the control group(P<0.01).The number of secondary surgery was less in the early warning group than that in the control group(P<0.05).Conclusion:Early warning intervention based on evidence-based cases can improve the recovery of gastrointestinal function,reduce the incidence rate of postoperative complications,restore the abilities of sucking and swallowing,and lower the secondary surgery in children with congenital intestinal atresia.
