摘要
目的:分析全产程分娩镇痛与第一产程分娩镇痛对妊娠高血压综合征(妊高征)产妇产程的影响。方法:选择2020年1-12月沈阳市妇婴医院研究生培养基地妇科收治的160例妊高征产妇作为研究对象,结合产妇的意愿进行分组,两组产妇均接受无痛分娩方式。观察组80例产妇采用全产程分娩镇痛方式,对照组80例产妇采用第一产程分娩镇痛方式,比较两组产妇产程、疼痛程度及患者满意度。结果:两组产妇在镇痛前、活跃期后期、第二产程及第三产程的疼痛程度评分比较,差异无统计学意义(P>0.05);对照组活跃期前期产妇的疼痛程度评分高于观察组,差异具有统计学意义(P<0.05);观察组产妇第一产程、第二产程的时间均短于对照组,差异具有统计学意义(P<0.05);观察组产妇及家属的满意度评价高于对照组,差异无统计学意义(P>0.05)。结论:全产程分娩镇痛与第一产程分娩镇痛比较,前者的镇痛效果更佳,缩短产程可有效维持产妇血压平稳与机体平稳状态,其应用价值较高。
Objective:To analyze the influence of labor analgesia in the whole process and labor analgesia at first stage on laborprocess of parturient women with pregnancy induced hypertension syndrome.Methods:A total of 160 parturient women withpregnancy induced hypertension syndrome treated in Department of Gynecology,Postgraduate Training Base of ShenyangWomen’s and Children’s Hospital from January 2020 to December 2020 were selected as the study subjects.They were dividedinto two groups according to the willing of the women.All parturient women received painless labor method.The 80 cases in theobservation group were given labor analgesia in the whole process of labor,and 80 cases in the control group were given laboranalgesia at the first stage of labor.The labor process,pain level and patient’s satisfaction in two groups were compared.Results:The pain evaluation scores of the two groups showed no statistically significant difference before analgesia,in the late active phase,at the second stage of labor and at the third stage of labor(P>0.05).In the early active phase,the pain evaluation score of thecontrol group was higher than that of the observation group,with statistically significant difference(P<0.05).The time of the firstand second stages of labor of the observation group was shorter than that of the control group,with statistically significantdifference(P<0.05).The satisfaction of parturient women and their families in the observation group was higher than that in thecontrol group,with statistically significant difference(P<0.05).Conclusion:Compared with labor analgesia at the first stage oflabor,labor analgesia in the whole process of labor has better analgesic effect.The shortening of labor process can effectivelymaintain the stable state of maternal blood pressure and body,and its application value is high.
