^(18)F-FDG PET/CT在结节病诊断中的应用价值 被引量:1

摘要 目的:探讨 ^(18)F-脱氧葡萄糖正电子发射计算机断层显像(^(18)F-FDG PET/CT)在结节病诊断中的价值。方法:回顾性分析 48 例经活检证实的结节病患者,其中胸部结节病 30 例,多系统结节病 18 例(全部有胸部侵犯)。所有患者均行 ^(18)F-FDG PET/CT 检查,同时行胸部 CT 平扫及增强扫描评价,对于病理证实的其他部位行 CT 平扫及增强扫描评价。结果:同活检证实部位做比较,检测活动性结节病的敏感性对于胸部、 腹腔-盆腔和骨骼-肌肉,^(18)F-FDG PET/CT 敏感性分别为 100%、87.5%和66.7%,CT 平扫及增强的敏感性分别为 100%、62.5%和 0%。CT 平扫及增强扫描和 ^(18)F-FDG PET/CT 扫描的敏感性分别为 78.6%和 88.1%。结论:^(18)F-FDG PET/CT 可获得完整的炎性活动部位的形态代谢图,有助于结节病的诊断,并帮助评价结节病累及范围,特别是不典型、复杂和多系统形式的结节病。 Objective:To evaluate the diagnostic value of ^(18)F-Deoxyglucose positron emission computed tomography (^(18)F-FDG PET/CT) in sarcoidosis.Methods:Forty-eight patients with sarcoidosis confirmed by biopsy were retrospectively analyzed,including 30 cases of thoracic sarcoidosis and 18 cases of extrthoracic sarcoidosis.All patients underwent ^(18)F -FDG PET/CT examination,as well as plain CT and enhanced CT evaluation of chest,and plain CT and enhanced CT evaluation of other sites confirmed by pathology.Results:Compared with biopsy confirmed sites,the sensitivities for detecting active sarcoidosis were 100%,87.5%,and 66.7% for chest,peritoneal -pelvic,and bone -muscle,respectively,and 100%,62.5%,and 0% for plain and enhanced CT,respectively.The sensitivity of CT and enhanced CT and ^(18)F-FDG PET/CT was 78.6% and 88.1%respectively,and improved to 84.6% and 94.9% after excluding 3 skin sites.Conclusion:^(18)F -FDG PET/CT can obtain a complete morphological and functional map of inflammatory active parts,which is helpful for the diagnosis of sarcoidosis and evaluation of the scope of sarcoidosis,especially the atypical,complex and multi-system forms of sarcoidosis.
