摘要
目的 探讨Caprini血栓风险评估量表在骨科术后患者静脉血栓风险预测中的应用效果。方法 采用回顾性病例对照研究,选取154例骨科术后静脉血栓(venous thromboembolism,VTE)形成患者作为病例组,选择同期骨科术后非VTE形成154例患者作为对照组。Caprini血栓风险评估量表的准确度及最佳临界值估计应用受试者工作特性ROC曲线(receiver operating characteristic curve)进行分析,并计算敏感度、特异度、阳性预测值、ROC曲线下面积(area under curve,AUC)、约登指数(youden’s index)。结果 Caprini血栓风险评估量表在骨科术后患者中应用ROC曲线的面积为0.762,95%置信区间为0.708~0.817,当临界值为8分时,约登指数最大值为0.49,灵敏度和特异度分别为75.97%和73.38%,VTE的阳性预测值为75.97%。结论 Caprini血栓风险评估量表对骨科术后患者VTE的发病风险具有一定的预测价值,Caprini量表的最佳临界值为8分时,量表的灵敏度和特异度均较高。
Objective To verify the validity of Caprini risk assessment scale in predicting the risk of venous thromboembolism(VTE) after orthopaedic surgery. Methods A case-control study was designed to retrospectively analyse the clinical data of 308 patients with VTE after orthopaedic surgery. One hundred and fifty-four VTE patients were selected as the case group,154 non-VTE patients hospitalized in the hospital at the same period were randomly selected as the control group. The basic data of patients of the two groups were retrospectively collected. The risk of VTE was assessed using the Caprini risk assessment scale. The validity and optimal threshold were analysed with the receiver operating characteristic curve for the sensitivity,specificity,positive predictive value,area under curve(AUC)and Youden’s index. Results The AUC of Caprini risk assessment scale was 0.762(0.708,0.817),the optimal threshold score was 8,the sensitivity and specificity of the scale were 75.97% and 73.38%,respectively. The positive predictive value of VTE >8 points was75.97%. Conclusions Caprini risk assessment scale has certain predictive value for the risks of VTE in patients after orthopaedic surgery.Anoptimal threshold at 8 gives higher sensitivity and specificity.
