摘要
目的 分析影响特发性血小板减少性紫癜(ITP)患儿预后的影响因素。方法 回顾性分析2018年5月至2020年5月在我院接受重组人血小板生成素治疗的96例ITP患儿的临床资料,根据患儿的预后情况进行分组。采用单因素和Logistic回归分析探讨预后的影响因素。结果 96例ITP患儿中,预后良好71例(73.96%),预后不良25例(26.04%)。预后不良组的发病年龄、病程、骨髓巨核细胞计数减少比例均高于预后良好组,丙种球蛋白使用比例低于预后良好组(P<0.05)。Logistic回归分析显示,发病年龄、病程、骨髓巨核细胞计数、丙种球蛋白使用情况是ITP患儿预后的独立影响因素(OR>1, P<0.05)。结论 ITP患儿预后情况与其发病年龄、病程、骨髓巨核细胞计数、丙种球蛋白使用情况密切相关,临床需对上述影响因素重点关注。
Objective To analyze the influencing factors of prognosis of children with idiopathic thrombocytopenic purpura(ITP).Methods The clinical data of 96 children with ITP who received recombinant human thrombopoietin therapy in our hospital from May 2018to May 2020 were retrospectively analyzed, and the children were grouped according to their prognosis. Univariate analysis and Logistic regression analysis were used to explore the influencing factors of prognosis. Results Among the 96 children with ITP, 71 cases(73.96%)had good prognosis and 25 cases(26.04%) had poor prognosis. The age of onset, disease duration, and proportion of bone marrow megakaryocyte count reduction in the poor prognosis group were higher than those in the good prognosis group, and the proportion of gamma globulin use was lower than that in the good prognosis group(P <0.05). Logistic regression analysis showed that age of onset, disease duration, bone marrow megakaryocyte count, and usage of gamma globulin were independent influencing factors of prognosis of children with ITP(OR >1, P <0.05). Conclusions The prognosis of children with ITP is closely related to the age of onset, disease duration, bone marrow megakaryocyte count, and usage of gamma globulin. Clinical attention should be paid to the above influencing factors.
