摘要
目的:探讨荧光原位杂交法(FISH)与免疫组化法(IHC)检测乳腺导管浸润癌人表皮生长因子受体2(HER2)表达的一致性分析。方法:选取2018年5月~2021年5月,乳腺导管浸润癌确诊患者106例。比较FISH法与IHC法检测乳腺导管浸润癌HER2检出情况。对FISH法与IHC法检测乳腺导管浸润癌HER2基因扩增和蛋白表达进行一致性分析。比较乳腺导管浸润癌病理特征与HER2蛋白表达的关系。结果:IHC检测结果:IHC染色0/1+ 者26例;2+ 者61例;3+ 者19例,阳性率75.47%(80/106)。FISH检测结果:IHC染色0/1+ 者1例扩增;IHC染色2+ 者8例扩增;IHC染色3+ 者18例扩增,阳性率为25.47%(27/106)。一致性比较采用Kappa值分析,IHC法检测HER2蛋白0、1+、2+、3+与FISH法检测 HER2基因扩增的kappa值分别为1.000、0.101、0.113、1.000。HER2蛋白表达与乳腺导管浸润癌组织学分级有关,且HER2蛋白表达与乳腺导管浸润癌组织学分级呈正相关,HER2蛋白表达与患者年龄、肿瘤生长位置、直径无关。结论:FISH法与IHC法可用于乳腺导管浸润癌HER2的检测,IHC法检测HER2蛋白结果为1+、2+时,利用FISH法复查效果最好。
Objective To explore the consistency between fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC) in the detection of human epidermal growth factor receptor 2(HER2) in invasive breast ductal carcinoma.Methods A total of 106 patients confirmed with invasive breast ductal carcinoma were enrolled between May 2018 and May 2021.The detection of HER2 by FISH and IHC was compared.The consistency between FISH and IHC in the detection of HER2 gene amplification and protein expression was analyzed.The relationship between pathological characteristics of invasive breast ductal carcinoma and the expression of HER2 protein was analyzed.Results IHC results:There were 26 cases with 0/1+ in IHC staining,61 cases with 2+ and 19 cases with 3+,with positive rate of 75.47% (80/106).FISH results:There was 1 case with 0/1+ amplification in IHC staining,8 cases with 2+ amplification and 18 cases with 3+ amplification,with positive rate of 25.47% (27/106).Kappa value analysis was applied in consistency comparison.The consistency kappa values between IHC detecting HER2 protein of 0,1+,2+ and 3+ and FISH detecting HER2 gene amplification were 1.000,0.101,0.113 and 1.000,respectively.The expression of HER2 protein was positively correlated with histological grading of invasive breast ductal carcinoma,while not correlated with patient’s age,growth sites and diameter of tumors.Conclusion Both FISH and IHC can detect HER2 in invasive breast ductal carcinoma.When there is HER2 protein of 1+ and 2+ detected by IHC,reexamination effect of FISH is the best.
