摘要
目的:分析儿童脓毒性休克接受激素与肺表面活性物质(PS)联合治疗的效果。方法:以我院2018年06月01日~2018年12月31日收治的100例脓毒性休克儿童为对象,按照治疗方法将患儿分为2组,1组接受激素治疗,视作对照组,1组接受激素与肺表面活性物质联合治疗,视作观察组,比较两组休克治愈率、疾病转归、心功能改善情况。结果:观察组治疗7天后休克恢复率为70%,高于对照组28%,观察组患儿在ICU住院时间明显短于对照组;观察组治疗3周后升压药停用率为72%,高于对照组48%,观察组病死率为14%,低于对照组32%;观察组升压药停用时间短于对照组,休克缓解时间及机械通气撤离时间均短于对照组;观察组治疗后出院时CI、LVEF结果均高于对照组,LVEDI、LVESI均低于对照组;观察组内出院时ALI患儿心功能CI、LVEF、LVEDI、LVESI指标水平高于ARDS患儿。结论:儿童脓毒性休克联合接受激素与肺表面活性物质治疗能够明显提升治愈率,改善患儿心功能,使患儿获得更好预后,应用价值突出。
Objective To analyze the effect of combined treatment with hormone and pulmonary surfactant (PS) in children with septic shock.Methods One hundred children with septic shock admitted to our hospital from June 01,2018 to December 31,2018 were divided into two groups according to the treatment method.One group received hormone therapy and was regarded as the control group.One group received hormone and pulmonary surfactant combined treatment,which was regarded as the observation group.The shock cure rate,disease outcome,and cardiac function improvement were compared between the two groups.Results After 7 days of treatment,the recovery rate of shock in the observation group was 70%,which was higher than that in the control group (28%).The hospital stay in the ICU was significantly shorter than that in the control group.The pressure-reduction rate in the observation group was 72 after 3 weeks.%,higher than the control group by 48%,the mortality rate of the observation group was 14%,which was lower than that of the control group (32%);the time of withdrawal of the booster in the observation group was shorter than that of the control group,the shock relief time and the mechanical ventilation evacuation time were both The expression of CI and LVEF in the observation group was higher than that in the control group after treatment.The LVEDI and LVESI were lower than the control group.The level of CI,LVEF,LVEDI,LVESI index of cardiac function in patients with ALI was higher than that in patients with ARDS at discharge.Conclusion Children with septic shock combined with hormone and pulmonary surfactant treatment can significantly improve the cure rate,improve the heart function of children,and make children with better prognosis,and the application value is outstanding.
