摘要
目的 研究凉山州某县HIV单阳家庭阴性配偶/固定性伴阳转状况及其影响因素,为进一步有效控制艾滋病疫情的传播和蔓延提供科学依据。方法 收集该县2017年1月至2018年12月艾滋病综合防治数据信息管理系统中的相关数据,建立回顾性队列研究。根据HIV单阳家庭阴性配偶/固定性伴阳转情况计算阳转率,对相关影响因素采用Log-rank检验进行单因素分析,用Cox比例风险模型进行多因素分析。结果 研究期间总共纳入单阳家庭1 372户,累计随访时间为967.45人年,94例阴性配偶/固定性伴发生HIV抗体阳转,2017-2018年总阳转率为9.72/100人年(95%CI:8.15~11.29)。相比未接受ART者,接受者HIV单阳家庭阴性配偶/固定性伴阳转的风险降低(HR=0.276,95%CI:0.169~0.451),相比先证者为男性,先证者为女性的阴性配偶/固定性伴阳转的风险升高(HR=2.810,95%CI:1.398~5.651)。结论 由于本研究未对阳转人群感染途径进行进一步的调查和检测,因此不能完全确定为配偶间传播,但凉山州某县HIV单阳家庭阴性配偶/固定性伴阳转率较高,应扩大ART覆盖面,加强艾滋病知识健康教育,积极采取有效的干预措施来控制该县疫情。
Objective To study the HIV seroconversion rate and risk factors of negative spouses/fixed sexual partners among HIV discordant couples in a county of Liangshan Prefecture.Methods Relevant data in the HIV/AIDS comprehensive response information management system(CRIMS) from January 2017 to December 2018 were collected to establish a retrospective cohort study.The seroconversion rate of negative spouses/fixed sexual partners among HIV discordant couples was calculated according to the seroconversion condition.The relevant risk factors were analyzed by univariate and multivariate using Log-rank test and Cox proportional hazard model.Results The cumulative follow-up time was 967.45 person-years.Among the 1372 discordant couples,94 HIV seroconversion spouses/fixed sexual partners were identified,so the seroconversion rate in 2017-2018 year was 9.72(95%CI:8.15-11.29) per 100 person-years.Compared with those without ART,the seroconversion risk of negative spouses/fixed sexual partners whose index cases with ART was reduced(HR=0.276,95%CI:0.169-0.451).Seroconversion risk of negative spouses/fixed sexual partners increased in women compared to men as index cases(HR=2.810,95%CI:1.398-5.651).Conclusions Since this study did not further investigate and detect the infection pathway of the seroconversion population,it cannot be identified as interspousal transmission.But the seroconversion rate of negative spouses/fixed sexual partners among HIV discordant couples in a county of Liangshan prefecture is high,so it is necessary to expand the coverage of antiretroviral therapy and strengthen the education of AIDS knowledge among HIV discordant couples to effectively control the status of infections in the county.
