摘要
目的探讨孕妇各期的维生素B_(12)营养状况,评估活性维生素B_(12)在孕妇维生素B_(12)营养状况中的诊断价值。方法取自2017年8月—11月本院妊娠早、中、晚期孕妇482例。检测血清中总维生素B_(12)、叶酸、活性维生素B_(12)、甲基丙二酸(MMA)含量,分析其在各组间差异,评估其在维生素B_(12)营养状况中的诊断效能。结果孕早期、孕中期、孕晚期血清总维生素B_(12)水平呈下降趋势(P<0.05);血清活性维生素B_(12)和MMA在各孕期未见明显变化(P>0.05)。活性维生素B_(12)与MMA具有较好的负相关性(r=-0.878,P<0.001);总维生素B_(12)与活性维生素B_(12)、MMA具有一定的相关性(r=0.589,P<0.001;r=-0.516,P<0.001)。活性维生素B_(12)和总维生素B_(12)对维生素B_(12)缺乏诊断的AUC分别为0.929和0.729,且活性维生素B_(12)与总维生素B_(12)的AUC比较差异有统计学意义(P<0.0001,Z=7.745)。结论血清活性维生素B_(12)对孕妇维生素B_(12)营养状况的评估具有较高价值和临床应用前景。
Objective To investigate the nutritional status of vitamin B_(12)in pregnant women at all stages of pregnancy,and assess the diagnostic value of active vitamin B_(12)in the nutritional status of vitamin B_(12)in pregnant women.Methods A total of482 pregnant women in our hospital from August to November in 2017 were recruited into this study to detect serum levels of total vitamin B_(12),folic acid,active vitamin B_(12),and methylmalonic acid(MMA).The differences of these markers were analyzed among the groups,so as to assess the diagnostic efficacy in vitamin B_(12)nutritional status.Results Total serum levels of vitamin B_(12)gradually decrease with gestational weeks in early,mid,and late pregnancy(P<0.05);and there was no significant difference in serum active vitamin B_(12)and MMA among the trimesters(P>0.05).Active vitamin B_(12)had a significant negative correlation with MMA(r=-0.878,P<0.001)while total vitamin B_(12)had a correlation with active vitamin B_(12)and MMA(r=0.589,P<0.001;r=-0.516,P<0.001).The AUC of active vitamin B_(12)and total vitamin B_(12)for the diagnosis of vitamin B_(12)deficiency were 0.929 and 0.729,respectively,and the difference in AUC between them was statistically significant(P<0.0001,Z=7.745).Conclusion Serum active vitamin B_(12)has high value and promising clinical application for the assessment of vitamin B_(12)nutritional status in pregnant women.
