摘要
目的探讨血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、抗凝血酶(AT-Ⅲ)、D-二聚体(D-D)、纤维蛋白原降解产物(FDP)、狼疮抗凝物筛选时间(LAS)、狼疮抗凝物确认时间(LAC)及狼疮抗凝物归化比率(LCR)在急性肺栓塞中的诊断价值。方法选取2020年1月—2021年6月在杭州师范大学附属医院治疗的80例急性肺栓塞症患者为观察组,同期80例健康人群为对照组。检测2组受试者凝血指标水平,分析其诊断急性肺栓塞的临床价值。结果诊断急性肺栓塞疾病ROC曲线下面积较大的是D-D、PT、AT-Ⅲ和FDP。9项指标联合诊断急性肺栓塞疾病的ROC曲线下面积为0.979,敏感度为90.0%,特异度为98.0%。结论9项指标与急性肺栓塞疾病发病相关,是诊断急性肺栓塞疾病的有效指标,联合诊断急性肺栓塞疾病检验效能更佳。
Objective To investigate plasma prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB),antithrombin(AT-Ⅲ),D-Dimer(DD),fibrinogen degradation products(FDP),and lupus anticoagulant related indicators lupus anticoagulant screening time(LAS),lupus anticoagulant confirmation time(LAC)and lupus anticoagulant naturalization rate(LCR)in acute pulmonary embolism.Methods A total of 80 patients with acute pulmonary embolism who were treated in the Affiliated Hospital of Hangzhou Normal University from January 2020 to June 2021 were selected as the observation group for the first onset,and 80 healthy people were selected as the healthy group during the same period.The coagulation index levels of the two groups of subjects were detected,and the clinical value of the diagnosis of acute pulmonary embolism was analyzed.Results Among the various indicators,the areas under the ROC curve for the diagnosis of acute pulmonary embolism are the largest in DD,PT,AT-Ⅲand FDP.The area under the ROC curve of D-D,PT,AT-Ⅲ,FDP,APTT,LAC,FIB,LCR and LAS in the combined diagnosis of acute pulmonary embolism disease was 0.979,the sensitivity was90.0%,and the specificity was 98.0%.Conclusion The nine indexes are related to the incidence of acute pulmonary embolism disease,and are effective indexes for the diagnosis of acute pulmonary embolism disease.The combined diagnosis of acute pulmonary embolism disease has better test efficiency.
