微创等离子消融术治疗小儿扁桃体肥大伴腺样体肥大对通气功能及氧化应激的影响 被引量:4

摘要 目的探讨微创等离子消融术治疗小儿扁桃体肥大伴腺样体肥大(AH)对通气功能及氧化应激的影响。方法选取平舆县人民医院2020年4月至2021年4月收治的扁桃体肥大伴AH患儿72例,随机分为实验组和对照组各36例。对照组采用常规扁桃体及腺样体切除术,实验组行微创等离子消融术治疗。比较两组患儿围手术期情况、术后通气功能变化、免疫功能、氧化应激水平及并发症发生情况。结果术中实验组出血量、术后视觉模拟评分法评分(VAS)及并发症发生率均低于对照组,且手术时间短于对照组(P<0.05);术后两组患儿双侧吸气总阻力(TIR)及呼气总阻力(TER)较术前均有降低,双侧鼻咽腔容积(NPV)较术前均有升高(P<0.05),通气功能组间比较差异无统计学意义(P>0.05);术后两组患儿免疫球蛋白(Ig)-G、Ig-M及Ig-A水平较术前均有降低(P<0.05),两组间差异无统计学意义(P>0.05);术后两组超氧化物歧化酶(SOD)较术前均有降低,实验组高于对照组(P<0.05),两组丙二醛(MDA)较术前均有提升,实验组低于对照组(P<0.05)。结论相较于传统切除治疗而言,微创等离子消融术治疗小儿扁桃体肥大伴AH在保证手术效果的基础上,可有效减少对患儿正常组织的损伤,减轻其术后疼痛水平,同时能尽可能降低手术带来的氧化应激反应,降低患儿术后并发症发生风险,安全性较高。 【Objective】To explore the effects of minimally invasive plasma ablation on ventilation function and oxidative stress in children with tonsil hypertrophy and adenoid hypertrophy(AH).【Methods】A total of 72 children with tonsil hypertrophy and AH admitted to the hospital between April 2020 and April 2021 were enrolled.They were randomly divided into experimental group and control group,36 cases in each group.The control group underwent routine tonsillectomy and adenoidectomy,while experimental group underwent minimally invasive plasma ablation.The perioperative conditions,changes of postoperative ventilation function,immune function,oxidative stress level and complications were compared between the two groups.【Results】The intraoperative blood loss,postoperative score of Visual Analogue Scale(VAS)and incidence of complications in experimental group were lower than those in control group,and operation time was shorter than that in control group(P<0.05).After surgery,bilateral total inspiratory resistance(TIR)and total expiratory resistance(TER)were decreased,while bilateral nasopharyngeal volume(NPV)was increased in both groups.There was no significant difference in ventilation function between the two groups(P>0.05).After surgery,levels of immunoglobulin(Ig)-G,Ig-M and Ig-A in both groups were decreased(P<0.05),but there was no significant difference between the two groups(P>0.05).After surgery,superoxide dismutase(SOD)in both groups was decreased,which was higher in experimental group than control group(P<0.05).After surgery,malondialdehyde(MDA)in both groups was increased,which was lower in experimental group than control group(P<0.05).【Conclusion】Compared with traditional resection,minimally invasive plasma ablation can effectively reduce the damage to normal tissues and alleviate postoperative pain while ensuring surgical effect in children with tonsil hypertrophy and AH,which can reduce oxidative stress response induced by surgery and risk of postoperative complications as far as possible,with hi
