摘要
目的:系统评价我国癌症术后病人疾病进展恐惧(fear of progression,FOP)的发生率及影响因素,为临床干预提供理论性指导。方法:通过计算机检索中英文数据库和手工检索,采用自由词和主题词相结合的方法,查找建库至2021年6月30日的公开发表的关于国内癌症术后病人疾病进展恐惧现况和影响因素的文献,由2名研究员独立筛选文献、提取资料并评价方法学质量,利用Stata 15.0进行Meta分析。结果:共纳入25篇文献(恐惧发生率18篇,影响因素25篇)。Meta分析结果显示:FOP发生率为51%[95%CI(0.41,0.61),P<0.001],影响癌症术后病人FOP的主要因素包括社会人口学因素、疾病相关因素、心理相关因素。结论:现有证据表明,癌症术后病人FOP发生率较高,FOP受年龄、文化程度、收入情况、癌症分期、复发转移情况、Gleason评分、症状困扰、健康指导、焦虑抑郁、希望水平、社会支持(主观支持、领悟社会支持)、疾病感知多种因素综合影响。
Objective:To systematically evaluate the incidence and influencing factors of disease fear of progression in cancer patients after surgery in China,so as to provide theoretical guidance for clinical intervention.Methods:Through computer retrieval of Chinese and English databases and manual retrieval,Literature on the status and influencing factors of disease progression fear of patients after cancer surgery in China published from inception to June 30,2021 was searched by using the method of combining free words and subject words.Two researchers independently screened the literature to extract data and evaluated the quality of the included methodology.Stata 15.0 was used for Meta-analysi.Results:A total of 25 articles were included(18 on the fear incidence and 25 on the influencing factors).Meta-analysis results showed that FOP incidence was 51%[95%CI(0.41,0.61),P<0.001].The main factors affecting FOP in patients after cancer surgery included sociodemographic factors,disease-related factors and psychological factors.Conclusions:Current evidence shows that the incidence of FOP is high in postoperative cancer patients,FOP is affected by age,education,income,staging,recurrence and metastasis,Gleason score,symptoms,health guidance,anxiety,depression,hope level,social support(subjective support,understanding of social support),disease perception,and multiple factors.
