摘要
目的探讨基于快速康复外科(ERAS)理念的揿针针刺治疗对混合痔患者疼痛的改善效果。方法以2020年7~12月于徐州市中医院接受治疗的76例混合痔患者为研究对象,按方便抽样法将其划分成2组,分别为对照组(38例)和观察组(38例)。对照组患者应用基于ERAS理念的护理干预,观察组患者应用基于ERAS理念的揿针针刺治疗。比较2组患者的疼痛程度、创面水肿严重程度、并发症发生率和临床症状消失时间。结果相较于对照组的并发症总发生率(23.68%),观察组(5.26%)明显更低(P<0.05)。2组在护理24 h、48 h、3 d和1周后的视觉模拟评分法评分和创面水肿严重程度评分均降低,且观察组均较同期对照组更低,差异均有统计学意义(P均<0.05)。观察组的水肿缓解时间、渗出物消失时间及创面愈合时间均早于对照组(P均<0.05)。结论基于ERAS理念的揿针针刺治疗可以有效缓解混合痔患者的疼痛程度,有助于促进创面水肿愈合,减少并发症的发生,加快康复进程。
Objective To investigate the effect of pressing needle therapy based on the concept of enhanced recovery after surgery(ERAS)on pain relief in patients with mixed hemorrhoids.Methods A total of 76 patients with mixed hemorrhoids who were treated at Xuzhou City Hospital of TCM from July to December 2020 were enrolled.They were allocated into two groups by convenience sampling,with the control group(38 cases)receiving nursing intervention based on the ERAS concept and the observation group(38 cases)receiving pressing needle therapy based on the ERAS concept.The pain intensity,severity of wound edema,incidence of complications,and disappearance time of clinical symptoms were compared between the two groups.Results The observation group had a significantly lower incidence of complications than the control group(5.26%vs.23.68%,P<0.05).After 24 hr,48 hr,3days,and 1 week of nursing,the scores of visual analogue scale and wound edema severity in the two groups were decreased,and the scores of the observation group were lower than those of the control group on the corresponding period(all P<0.05).The edema relief time,exudate disappearance time,and wound healing time in the observation group were earlier than those in the control group(all P<0.05).Conclusion Pressing needle therapy based on the ERAS concept for patients with mixed hemorrhoids can effectively relieve their pain,promote the healing of wound edema,reduce the occurrence of complications,and speed up the recovery process.
