摘要
目的:分析桡骨远端骨折患者实施中医正骨手法与中医方剂(桃红四物汤加减)联用治疗对血清炎性因子、骨折修复细胞因子、腕关节功能恢复效果的影响。方法:选定汕头市中心医院骨科2018年4月-2020年4月收治的70例桡骨远端骨折患者,以双盲随机抽样法分组(每组样本容量35例),对照组采纳闭合复位夹板外固定术治疗,观察组在对照组基础上采纳中医正骨手法联合桃红四物汤加减治疗,两组均在治疗4周后观察治疗效果,对比两组临床疗效、血清炎性因子[肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、白细胞介素(Interleukin,IL)-6、IL-1]水平、骨折修复细胞因子[转化生长因子-β1(Transforming Growth Factor-β1,TGF-β1)、血管内皮生长因子(Vascular Endothelial Growth Factor,VEGF)、骨形态发生蛋白2(Bone Morphogenetic Protein 2,BMP-2)]水平、Cooney腕关节功能评分。结果:观察组临床总有效率比对照组高(P <0.05)。观察组治疗7 d后血清TNF-α、IL-6、IL-1因子水平均比对照组低(P <0.05)。观察组治疗7 d后血清TGF-β1、VEGF、BMP-2因子水平比对照组高(P <0.05)。观察组治疗7 d后Cooney腕关节功能评分均比对照组高(P <0.05)。结论:中医正骨手法联合桃红四物汤可有效改善桡骨远端骨折患者的腕关节功能,缓解患肢疼痛、肿胀等症状,抑制TNF-α、IL-6、IL-1等炎症介质释放,促进骨折修复,弥补了单纯闭合复位夹板外固定术治疗的不足和缺陷,疗效确切、显著,值得借鉴。
Objective:To analyze the effect of traditional Chinese medicine bone setting manipulation plus traditional Chinese medicine prescription(the Taohong Siwu decoction)(桃红四物汤加减)on serum inflammatory factors,fracture repair cytokines and wrist joint function recovery in patients with distal radius fracture.Methods:70 patients with distal radius fracture from April 2018 to April 2020 were selected and divided into two groups by double-blind random sampling method(35 cases in each group).The control group was treated by closed reduction and splint external fixation,while the observation group was treated additionally by TCM bone setting manipulation plus the Taohong Siwu decoction.The therapeutic effect in the two groups was observed after 4 weeks of treatment.The clinical efficacy,serum inflammatory factors(TNF-α,IL-6,IL-1)levels,fracture repair cytokines(TGF-β1,VEGF,BMP-2)levels,Cooney wrist function score were compared.Results:The clinical total effective rate in the observation group was higher than the control group(P<0.05).The serum levels of TNF-α,IL-6 and IL-1 in the observation group were lower than those in the control group after 7 days of treatment(P<0.05).The serum levels of TGF-β1,VEGF and BMP-2 factors in the observation group were higher than those in the control group after 7 days of treatment(P<0.05).After 7 days of treatment,Cooney wrist function score in the observation group was higher than the control group(P<0.05).Conclusion:TCM bone setting manipulation plus the Taohong Siwu decoction can effectively improve wrist joint function of patients with distal radius fracture,relieve limb pain and swelling,inhibit the release of TNF-α,IL-6,IL-1 and other inflammatory mediators,promote fracture repair,and make up for the deficiency and defects of simple closed reduction and splint external fixation.The curative effect is exact and significant,which is worthy of reference.
