摘要
目的研究核心稳定性训练+深层肌肉刺激运用在腰椎间盘突出症中的价值及对患者术后功能恢复的影响。方法选择医院2018年8月—2019年5月纳入的96例腰椎间盘突出症患者,依据随机数字表法分成两组各48例,研究组给予核心稳定性训练+深层肌肉刺激,对照组给予深层肌肉刺激,对两组总有效率进行评价,分别在干预前后进行临床症状积分、视觉模拟评分(visual analogue scale/score,VAS)、日本骨科协会评估治疗评分(Japanese orthopaedic association scores,JOA)、功能障碍指数(Oswestry disability index,ODI)评价,并测定腰背伸肌群生物力学性能(平均功率、腰背伸状态下峰力矩、腰背屈/伸比值)、炎性因子[白介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)],随访1年,记录两组复发率,比较两组干预结果。结果研究组总有效率93.75%高出对照组77.08%,差异有统计学意义(P<0.05)。干预前两组的临床症状积分、VAS、JOA、ODI评分、腰背伸肌群生物力学性能、炎性因子对比差异无统计学意义(P>0.05),干预后研究组各项指标均优于对照组,差异有统计学意义(P<0.05)。研究组1年复发率4.17%低于对照组16.67%,差异有统计学意义(P<0.05)。结论核心稳定性训练+深层肌肉刺激运用在腰椎间盘突出症中效果突出,可有效减轻患者疼痛,并促进相应症状消失,增强腰椎功能,改善腰背伸肌群生物力学性能,消除炎性反应,恢复日常生活能力,防止复发,值得推广。
Objective To study the impact of core stability training plus deep muscle stimulation on the postoperative functional recovery of lumbar intervertebral disc protrusion patients.Methods A total of 96 lumbar intervertebral disc protrusion patients treated from August 2018 to May 2019 in our hospital were selected and assigned to two groups according to random number table,48 cases in each group.The research group was adopted the core stability training and deep muscle stimulation;the control group was adopted the deep muscle stimulation.The total effective rate of the two groups was evaluated,and the clinical symptom scores,visual analogue scale/score(VAS),Japanese orthopaedic association scores(JOA),and Oswestry disability index(ODI)were evaluated,and measured the biomechanical properties of lumbar extensor muscles(average power,peak torque in lumbar dorsi extension,lumbar dorsiflexion/extension ratio),inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],followed up for 1 year,recorded the recurrence rate of the two groups,and compared the intervention results of the two groups.Results Total effective rate in the research group(93.75%)was significantly higher than control group(77.08%),the difference was statistically significant(P<0.05);before intervention,symptom scores,VAS,JOA,ODI score,biomechanical properties of low back extensors and inflammatory factors between groups were not significantly different(P>0.05);after intervention,related scores in the research group were better than control group,the difference was statistically significant(P<0.05);the one-year recurrence rate in the research group(4.17%)was significantly lower than control group(16.67%),the difference was statistically significant(P<0.05).Conclusion Core stability training+deep muscle stimulation is effective in lumbar disc herniation,which can effectively relieve pain in patients,promote the disappearance of corresponding symptoms,enhance lumbar spine function,improve the biomechanical properties of lumbar extensor musc
