摘要
目的探讨丝苏氨酸蛋白激酶1(Nuclear Dbf2 p-related kinase 1,NDR1)和微小核糖核酸(micro RNA,miR)-146a在呼吸道合胞病毒(Respiratory syncytial virus,RSV)感染患儿外周血清中的表达情况及临床意义。方法收集2019年7月~2020年5月西北大学附属医院(西安市第三医院)检验科、西安市儿童医院就诊的77例RSV感染患儿作为研究对象。按照病情严重程度分为重症组36例,轻症组41例。同时选取53例同期来医院体检的健康儿童为对照组。采用实时荧光定量PCR(qRT-PCR)法检测外周血清中miR-146a表达水平;酶联免疫吸附法(ELISA)检测NDR1水平;Pearson法分析NDR1与miR-146a表达的相关性及二者与相关炎性因子表达的相关性。受试者工作特征曲线(ROC)评估血清NDR1和miR-146a水平对RSV感染严重程度的诊断价值。结果与对照组相比,RSV感染患儿外周血中NDR1水平(9.95±2.38 pg/L vs 6.46±1.12 pg/L)下调,miR-146a表达水平(1.03±0.17 vs 2.12±0.43)上调,差异具有统计学意义(tmiR-146a=17.519,tNDR1=11.204,均P<0.05)。RSV感染重症组、轻症组及对照组中hs-CRP,IL-4,IL-5,IL-6,IL-2和IFN-γ比较差异均有统计学意义(F=67.825,564.775,256.06,202.992,77.579,320.470,均P<0.05)。RSV感染患儿血清NDR1水平与IL-2,IFN-γ表达均呈正相关(r=0.473,0.620,均P<0.05),与hs-CRP,IL-4,IL-5,IL-6水平呈负相关(r=-0.259,-0.559,-0.422,-0.505,均P<0.05);miR-146a与hs-CRP,IL-4,IL-5,IL-6水平均呈正相关(r=0.381,0.636,0.421,0.570,均P<0.05),与IL-2,IFN-γ水平呈负相关(r=-0.646,-0.744,均P<0.05);RSV感染患儿外周血清中NDR1与miR-146a表达水平呈负相关(r=-0.225,P<0.05);ROC结果显示,血清NDR1,miR-146a水平预测重症RSV感染患儿的曲线下面积(AUC)分别为0.911(95%CI:0.823~0.964),0.741(95%CI:0.628~0.834),敏感度分别为82.93%,68.29%,特异度分别为88.89%,75.00%,二者联合预测重症RSV感染患儿的AUC为0.925(95%CI:0.842~0.973),敏感度和特异度分别为90.24%和83.33%。结论NDR1在RSV感染患儿外周�
Objective To investigate the expression and clinical significance of Nuclear Dbf2p-related kinase 1(NDR1)and micro RNA(miR)-146a in the peripheral serum of children infected with Respiratory syncytial virus(RSV).Methods Blood samples of children with RSV infection in the Affiliated Hospital of Northwest University/Xi’an No.3 Hospital,Xi’an Children’s Hospital from July 2019 to May 2020 were collected.According to the severity of the disease,there were 36 cases in the severe group and 41 cases in the mild group.At the same time,53 healthy children who came to the hospital for physical examination during the same period were selected as the control group.The expression level of miR-146a in peripheral blood was detected by real-time fluorescence quantitative PCR(qRT-PCR).NDR1 was detected by enzyme-linked immunosorbent assay(ELISA).The correlation between the expression of NDR1 and miR-146a and the expression of related inflammatory factors was analyzed by Pearson method.Receiver operating characteristic curve(ROC)was used to assess the diagnostic value of serum NDR1 and miR-146a levels for the severity of RSV infection.Results Compared with the control group,the level of NDR1(9.95±2.38 pg/L vs 6.46±1.12 pg/L)in peripheral blood of children infected with RSV was significantly down-regulated,and the expression level of miR-146a(1.03±0.17 vs 2.12±0.43)was up-regulated.All were statistically significant(tmiR-146a=17.519,tNDR1=11.204,all P<0.05).There were significant differences in hs-CRP,IL-4,IL-5,IL-6,IL-2 and IFN-γin RSV infection severe group,mild group and control group(F=67.825,564.775,256.06,202.992,77.579,320.470,all P<0.05).The serum NDR1 level in children with RSV infection was positively correlated with IL-2 and IFN-γexpression(r=0.473,0.620,all P<0.05),and negatively correlated with hs-CRP,IL-4,IL-5 and IL-6 levels(r=-0.259,-0.559,-0.422,-0.505,all P<0.05);MiR-146a was positively correlated with hs-CRP,IL-4,IL-5 and IL-6 levels(r=0.381,0.636,0.421,0.570,all P<0.05),and negatively correlated with
