摘要
目的 了解助产士强化沟通模式对产妇分娩应激表现及产程的影响。方法 随机选择2017年11月—2019年1月在该院分娩的100例产妇作为研究对象,并给予随机数表分组,常规护理服务组与助产士强化沟通模式组各50例,分别予以常规护理及助产士强化沟通模式,对两组产后出血情况、产程平均时间、妊娠结局、产妇应激表现、产妇满意度进行调查与评估。结果 助产士强化沟通模式组各个时段出血量明显少于常规护理服务组,差异有统计学意义(P<0.05)。助产士强化沟通模式组护理后的产妇满意度为100.0%,高于常规护理服务组的80.0%,差异有统计学意义(χ^(2)=11.111,P<0.05)。助产士强化沟通模式组产程时间短于常规护理服务组,差异有统计学意义(P<0.05)。护理后,两组产妇焦虑应激、抑郁应激评分均显著低于护理前,且助产士强化沟通模式组低于常规护理服务组,差异有统计学意义(P<0.05)。对比常规护理服务组,助产士强化沟通模式组母乳喂养率明显较高,产褥感染发生率明显较低,盆腔粘连发生率明显较低,剖宫产率明显较低,差异有统计学意义(P<0.05)。结论 对阴道分娩产妇实施助产士强化沟通模式干预,可有效减少产后出血量,缩短产程,促进分娩应激反应减轻,有利于构建良好的护患关系。
Objective to understand the effect of midwives'intensive communication mode on maternal stress performance and labor process.Methods 100 puerperae who gave birth in this hospital from November 2017 to January 2019 were randomly selected as the research subjects,and assigned to random number table grouping,with 50 cases in each group of routine nursing service group and enhanced communication mode of midwives group.Routine nursing and midwives were given intensive communication mode,respectively,to investigate and evaluate the postpartum hemorrhage,average labor time,pregnancy outcome,maternal stress performance,and maternal satisfaction in the two groups.Results The blood loss in the midwives intensive communication mode group was significantly less than that in the routine nursing service group in each period,and the difference was statistically significant(P<0.05).The maternal satisfaction after nursing in the midwife intensive communication mode group was 100.0%,which was higher than that in the routine nursing service group 80.0%,and the difference was statistically significant(χ^(2)=11.110,P<0.05).The duration of labor in the midwife intensive communication mode group was shorter than that in the routine nursing service group,and the difference was statistically significant(P<0.05).After nursing,the maternal anxiety stress and depression stress scores in the two groups were significantly lower than those before nursing,and the midwife intensive communication mode group was lower than the routine nursing service group,the difference was statistically significant(P<0.05).Compared with the routine nursing service group,the midwife intensive communication mode group had a significantly higher breastfeeding rate,a significantly lower incidence of puerperal infection,a significantly lower incidence of pelvic adhesions,and a significantly lower cesarean section rate,the difference was statistically significant(P<0.05).Conclusion The intervention of midwives intensified communication mode for vaginal delivery mothe
