摘要
目的 探讨影响超选择血管内栓塞治疗儿童大型血管瘤的临床效果的因素。方法 回顾性分析82例儿童大型血管瘤患儿临床资料,分析超选择血管内栓塞治疗儿童大型血管瘤的临床疗效的影响因素。结果 儿童大型血管瘤患儿治疗总有效率为87.80%(72/82),其中CR 55例,PR 14例,MR 3例,NC 10例。单因素分析显示:年龄、供血动脉数目、供血动脉来源、动静脉瘘(VAF)与超选择血管内栓塞治疗儿童大型血管瘤的临床疗效有关,差异有统计学意义(P<0.05);性别、瘤体直径、栓塞材料与超选择血管内栓塞治疗儿童大型血管瘤的临床疗效无关,差异无统计学意义(P>0.05)。多因素分析显示:年龄<5月(β=2.118,OR=8.318,95%CI=2.013~34.370)、供血动脉数目≥3支(β=2.197,OR=9.000,95%CI=1.084~74.756)、供血动脉来源混合(β=2.360,OR=10.590,95%CI=2.411~46.512)、VAF(β=3.983,OR=53.667,95%CI=9.058~317.970)是影响超选择血管内栓塞治疗儿童大型血管瘤的临床疗效的高危因素(P<0.05)。结论 超选择血管内栓塞治疗儿童大型血管瘤的临床效果受年龄、供血动脉数目、供血动脉来源、VAF影响。
Objective To investigate the factors influencing the clinical efficacy of hyperselected endovascular embolization in the treatment of large hemangioma in children.Methods The clinical data of 82 children were analyzed retrospectively,and the influencing factors were analyzed.Results The overall treatment efficiency of children with large hemangioma was 87.80%(72/82),including 55 C R,14 P R,3 M R,and 10 N C.Univariate analysis showed that age,number of blood arteries,source,arteriovenous fistula(VAF) were related to the clinical efficacy of large hemangioma in children,with statistical difference(P<0.05);sex,tumor diameter and embolic material were not related with the clinical efficacy of the superintravascular embolization,and there had no statistical difference(P>0.05).Multivariate analysis showed that the age was <May(β=2.118,OR=8.318,95%CI=2.013~34.370),3 total number of blood supply arteries(β=2.197,OR=9.000,95%CI=1.084~74.756),blood supply artery source mix(β=2.360,OR=10.590,95%CI=2.411~46.512),VAF(β=3.983,OR=53.667,95%CI=9.058~317.970) was a high risk factor affecting the clinical efficacy of superendovascular embolization for large hemangioma in children(P<0.05).Conclusion The clinical effect of endovascular embolization is affected by age,number of blood arteries,source of blood arteries and VAF.
